Job Description The University of California, San Francisco (UCSF) Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center seeks a full-time Advanced Practice Provider (APP) to treat mechanical circulatory support (MCS), heart transplant, and advanced heart failure patients, among others. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers, caring for patients with advanced heart failure across the continuum of care. Our Advanced Heart Failure & MCS programs are nationally known for cutting edge advanced heart failure care and support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The APP serves as primary provider in the hospital setting for these patients and will work closely with the Advanced Heart Failure Cardiologist, Cardiothoracic Surgery and Intensive Care teams along with nursing, case management, social workers and other multidisciplinary teams to evaluate, treat and manage this highly complex patient population in the Intensive Care and Acute Care Units. Responsibilities RESPONSIBILITIES 60% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1. Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Job Description Under the supervision of the LVN Supervisor/Nurse Manager, the Licensed Vocational Nurse - Pediatric Dermatology provides direct nursing care for pediatric patients in Ambulatory Pediatric Specialties practices, with primary assignment supporting Pediatric Dermatology. Coordinates outpatient care, performs patient intake, obtains vital signs, reconciles medications, administers medications and immunizations, performs laboratory procedures, assists providers during examinations and procedures, coordinates care between patients, families and the interdisciplinary team, documents care in Epic, and promotes safe, high-quality patient care while adhering to UCSF policies and Living PRIDE standards. The Pediatric Dermatology clinic cares for infants, children, and adolescents with common and rare skin disorders. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Answers patient call lights, telephone, and paging system and responds as appropriate. Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold. Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening milk container, cutting food for patient. Knowledge/Skills/Abilities/Expectations Must read, write and speak fluent English.
