SUPERVISION • Meet with unit personnel, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Review and evaluate assigned personnel and make written evaluations to the RN Supervisor or Assistant Director of Nurses. • Coordinate Nursing service with other departments and nursing units. • Notify the attending physician, RN Supervisor and Director of Nurses when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with the facility’s established policies and procedures. • Assure that a stock of medications, medical supplies, equipment, etc., is maintained at all times. Requisition equipment and supply needs as required. • Initiate investigation of accidents and unusual occurrences and make the necessary written report to the Director of Nurses as established in the facility’s policies and procedures. • Notify the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident has a significant change of condition or becomes critically ill or injured, as outlined within facility’s established policies. • Work with the facility’s consultants as necessary and implement recommended changes as directed by the RN Supervisor of the Director of Nurses. • Investigate complaints and grievances and make necessary oral/written reports to the RN Supervisor or Director of Nurses. • In cases of death, notify the funeral home and ensure that the body is properly prepared/cleaned for pick-up. • Evaluate emergencies and determine emergency measures to be used, including the use of chemical or mechanical restraints. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Provide skin care to prevent breakdown, assist with safe ambulation, transfers, and prescribed exercises, ensuring a safe and supportive environment. Monitor and record vital signs as directed. Document and immediately report any changes in the patient’s physical, mental, or emotional condition to the Case Manager or Supervisor. Use the HospiceMD EMR platform to log patient visits, care plans, and clinical narratives accurately and timely, in accordance with agency policy and regulatory standards. Travel reliably between assigned patient locations across the SGV territory. Track business travel and accurately submit monthly mileage invoices using Concur for reimbursement. Work collaboratively with nurses, social workers, chaplains, and families to carry out the comprehensive plan of care. Provide emotional support and companionship to patients and their loved ones. Additional duties as assigned. Qualifications Active California Certified Home Health Aide (CHHA) license in good standing. Current and valid CPR certification. Proficiency in Mandarin or Cantonese is strongly preferred to best serve our diverse patient demographics in the SGV area, though it is not strictly required. Fluent verbal and written English communication is required. Strong computer knowledge with previous hands-on experience using Electronic Medical Records (EMR) software, specifically HospiceMD, and comfort with digital expense tools like Concur. Valid driver’s license, a reliable and well-maintained vehicle, and active auto liability insurance to meet mandatory field travel requirements. Deep empathy for the ill and elderly, a calm demeanor around end-of-life care, high integrity, and strict adherence to HIPAA patient confidentiality regulations. Lifting & Mobility: Ability to lift, push, pull, or move up to 50 pounds frequently. Ability to safely stand, walk, bend, stoop, kneel, and assist patients with varying physical mobility levels throughout an 8-hour field shift. Environmental Job Requirements and Working Conditions This role is a part-time field-based position. The home office is located at 1668 S. Garfield Ave. 2nd Floor, Alhambra, CA 91801. The national target pay range for this role is $27.00 - 36.00 per hour. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors. Job Summary The Staff RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame. New Grad Program. One year critical care experience is preferred. Excellent customer service skills required. Minimum License Requirement: Current RN license in the State of California. BLS certification required. ACLS required within three (3) months of hire/transfer. National Institutes of Health (NIH) Stroke Scale Certification "A" required within three (3) months of hire/transfer for those working at ICH and FPH. Annual one (1) hour of Stroke Education required for those working at QVH. Must be available to work every other weekend Delivering world-class health care one patient at a time.
