Job Description The Department of Perioperative Services at SUNY Downstate Health Sciences University is seeking a full-time Registered Nurse 2, Operating Room. Distinguishing Characteristics Advanced performance level; provides direct nursing care to individuals in an acute care environment requiring advanced clinical interventions; performs professional nursing functions in three areas: direct individual care, preceptor, and clinical management. Illustrative Duties Direct Individual Care Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Implements nursing interventions in response to changes in individual status and unusual circumstances. Interprets assessment data to determine if individuals are at physiological or psychosocial risk. Takes steps to decrease stress and increase the effectiveness of individual coping mechanisms; uses effective communication strategies with individuals, families, staff, and the general public. Assesses individual status upon entry to the hospital and throughout hospitalization. Identifies implications of individuals’ statuses and prioritizes data collection based on individuals’ immediate needs. Identifies safety and security risks to promote a culture of safety and to prevent injury and harm to individuals, visitors, and staff. Preceptor Guides lower-level, and new to specialty, registered nurses in choosing nursing interventions, developing skills, and applying knowledge. Serves as a resource nurse for staff members, orienting new staff and providing feedback on nursing skills for specialty care areas. Develops and maintains care standards knowledge bases for specialty care areas and develops care guidelines for unit-specific individual populations. Maintains a minimum standard of continuing education for specialty areas. Evaluates and assesses individual staff and nursing student development needs. Counsels and implements a variety of teaching strategies to meet those needs in consultation with nursing management staff. Participates in in-service and other educational programs related to clinical specialties. May identify, develop, and implement nursing research studies on clinical units, in consultation with medical staff consultants. Clinical Management Consults and coordinates with other health care team members to resolve complex clinical issues and develop short and long-term clinical goals for assigned units. Integrates and evaluates clinical care knowledge specific to the needs of unit individuals; develops unit-specific policies and procedures in consultation with nursing management staff. Identifies and implements corrective actions to improve the quality of individual care; may oversee equipment testing and supplies. Leads and attends various hospital committees. Assists Registered Nurse Supervisor 1 (T&RC) staff with implementing and achieving unit operating changes; may take charge of a unit in the absence of a Registered Nurse Supervisor 1 (T&RC). Resolves individual relation issues. Identifies and reports issues related to individual care, operation, and quality outcomes. Identifies potential conflict situations and facilitates solutions. Qualifications Required Qualifications Promotion: possession of a license and current registration as a registered professional nurse in New York State and 1-year of service as a Registered Nurse 1 (T&RC). Open Competitive: possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience in the acute care specialty area. Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Establishes and maintains a therapeutic relationship with the patient and family. 3. Uses evidence-based practices to increase understanding of patients perceptions of care. 4. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed. 5. Supports staff use of evidence-based practices to increase understanding of patients perceptions of care. 7. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager. 4. Demonstrates patient focused and cost effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Professional Development 1. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Working knowledge of all infection control procedures and safety precautions. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. Responsibilities We are actively hiring for an Registered Nurse (RN) PreOp PACU. Promote the mission, vision, and values of SCA . Assess patient’s preoperative and postoperative status at intervals according to facility or health care organization policies and procedures to include the patient’s cardiovascular system, respiratory system and airway management needs, neurological system, hemodynamic stability, pain and comfort needs, and mental status. Interpret information accurately from various noninvasive and invasive monitors. Describe the underlying pathophysiology of commonly encountered disease processes. Describe the physiology associated with various anesthesia techniques and surgical or procedural interventions. Apply the relevant scoring system/discharge criteria for patient transfer ( e.g. , Aldrete Score, Modified Aldrete Score [ i.e. , PARSAP], Post Anesthesia Discharge Scoring System [PADSS], White) score accurately as defined by organizational policy. Develop an individualized perianesthesia plan of care. Implement the plan of care including the following interventions: Preparing for and managing patient recovery from anesthesia – general. Preparing for and managing patient recovery from anesthesia – monitored anesthesia care. Preparing for and managing patient recovery from anesthesia – regional. Preparing for and managing patient recovery from anesthesia – local. Preparing for and protecting patient from injury caused by positioning, thermal sources, and extraneous objects. C ommunicating the patient’s current status throughout the perianesthesia continuum of care. P roviding acute pain and comfort management as needed. A dministering medications safely and correctly by using the 8 -rights of safe medication administration. P erforming interventions to maintain the integrity of the patient’s wound and tissue perfusion at or above baseline levels . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems, and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect patient from infection. M anaging patient’s other co-morbid conditions as applicable. M anaging patient’s fluid as prescribed. A ssessing knowledge level of patient or designated support person and providing education regarding the expected psychosocial response, nutritional management, medication management, pain and comfort management, wound management, and expected responses to the operative or invasive procedure; I nvolving patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice; and P roviding consistent and comparable care regardless of the setting. Evaluate the patient’s progress toward attaining outcomes. Transfer care as needed. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions. Document nursing care completely, accurately, and legibly. Collaborate effectively with other disciplines as needed. Participate in quality review and performance improvement projects. Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 days of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advanced Life Support (PALS) course is required when pediatric care is included in the facility scope services within 90 days of employment or documentation of current PALS certification.
