Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings. Ability to engage and communicate effectively with elderly and chronically ill patients and their families. Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target. Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus. Key Responsibilities: As the In-House Nurse Practitioner , you will provide essential care and leadership within the facility, including: Conducting new admission evaluations and monthly stability visits for long-term residents Performing comprehensive physical examinations and evaluating changes in patient conditions Prescribing medications and treatments as needed Developing patient care policies and facilitating referrals to specialists Coordinating advanced care planning and discharge visits Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups Supporting the physician-patient relationship and addressing gaps in care Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
