Job Description Job Function Summary: Involves the provision of integrated and accessible primary and specialty patient care services in a clinic or center environment. Care is provided by clinicians and assistants who are accountable for addressing a large majority of personal health care needs and developing a sustainable partnership with patients. Ensures optimal patient outcomes through effective leadership and / or provision of patient care delivery in healthcare clinics or centers. Promotes shared decision-making at all levels to ensure a shared voice in practices, exchange of ideas, enhanced communication, and continually improved patient services. Management levels may also be responsible for clinic operations. (Note, when primary focus is clinical operations, see Ambulatory Administration job function.) Manages the delivery of ambulatory patient care, which typically includes nursing, and may include laboratory, radiology, dental, etc. Facilitates the flow of patient care activities; may also participate in practice or center management operations. Qualifications Required qualifications: At least 5 years of clinical experience in nursing with 2 of those years in a dermatology setting. Minimum of 2 years of nursing supervisory experience in an outpatient clinic familiar with Dermatology diseases. Strong interpersonal skills to create and maintain a climate for effective communication and collaboartive decision making. Able to convey information with authority, handle highly sensitive matters, and interact with high level contacts from other departments/organizations. Strong verbal and written communication skills and ability to explain technical specialty concepts thoroughly. Strong leadership, fiscal, and operations management skills, with the ability to collaborate effectively with physicians, management, and staff from all disciplines. Ability to create and maintain a goal-oriented climate of teamwork, collaboration, and consistent achievement of objectives. Strong analytical and critical thinking skills, with the ability to quickly analyze problems, determine the appropriate level of intervention, and develop and apply effective solutions. Highly devleoped personal organizational skills. Proven ability to organize staff and tasks, manage mulitple priorities/projects through completion and on schedule, oversee multiple locations, and delegate work functions in an efficient and effective manner in a complex and challenging environment. Flexibility to adjust to constantly changing priorities. Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources issues and performance. Demonstrated knowledge of relevant regulatory requirements, as well as related legislative, accreditation, licensing, and compliance environments. Maintains current knowledge of nursing practices, dermatologic advancements, new methodologies, relevant industry/regulatory compliance, accreditation requirements, medical center health and safety guidelines, and advances in the field of specimen processing, automation, and procedures. Strong knowledge of quality assurance methods and ability to apply principles to design quality systems. Ability to use laboratory applications/clinical softwares/databases/dashboards. Skills to perform data checks using queries, visual review, and other appropriate means to verify data accuracy. Preferred Requirements Master’s degree in nursing or equivalent combination of experience/training. Experience in large academic medical setting and ambulatory care. Experience using EMR-EPIC/APeX. Job Description The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication indications and the importance of adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure.This position will rotate through the Diuretic Clinic as needed with responsibilities including initiating IV access, blood draws, administration of IV medications, review of laboratory results, and follow-up patient assessment. Job Description The Sr Licensed Vocational Nurse, under the supervision of the Nurse Manager, guided by the procedures of the Ambulatory Services and the Ambulatory Services Quality Nursing Division, and in collaboration with the physician staff, had the clinical responsibility for developing and maintaining a high standard of patient care, under the Urology Procedure Center and Urology Faculty Practice. They participate in the planning for the provision of patient care. They provide patient health education teaching for Urology patients across the lifespan, catheter teaching and management skills and clinical support. They collaborate with the Nurse Manager, Administrative Director, and Medical Director to develop monitor and maintain systems pertaining to Urology patients. They are responsible for maintaining a safe environment for patients and staff, and for daily operations. All components of nursing care will be individualized to meet he patient’s physiological and psychosocial developmental level and needs. The Licensed Vocational Nurse consistently projects a professional and courteous demeanor and shall comply with the Medical Center House and Telephone Standards. The Sr Licensed Vocational Nurse will be primarily located at Bayfront, however, must have flexibility to cover multiple UCSF Medical Center sites. Qualifications Required qualifications: Minimum of 3 years of LVN experience Must have 3-5 years of related experience with procedures such as foley catheter insertion Must be able to do medication refills, medication and equipment prior authorizations and disability applications Excellent EPIC skills This position requires flexibility to orient and work at all UCSF Medical Center locations.
