Maintain effective communication with other members of the interdisciplinary team utilizing facility processes in order to assure the highest standard of care is maintained for each resident assigned to his or her care. Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures. Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services. Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Position Requirements Associate's Degree from credentialed school of nursing. Description The Nursing Attendant is an individual who functions in a support role to the Registered Professional Nurse by providing direct patient care and related activities as assigned. Responsibilities Assists the nursing staff in performing selected nursing procedures, treatments, and functional tasks to specific patients including vital signs: Temperature, Pulse, Respirations, Blood Pressure and Pain Rating. Performs procedures associated with activities of daily living, ambulation, mobility, and personal hygiene of the patient. Observes, reports, and records routine and specific patient data, behaviors and responses to treatment on the appropriate flow sheet/EMR. Reports pertinent information and observations regarding patients to the registered nurse Performs procedures related to assuring a safe environment for patients, staff and others. Performs procedures associated with the admission, transfer and discharge of patients. Performs procedures associated with the maintenance of food and fluid intake. Performs procedures associated with excretion of body waste and or loss of fluid. Provides for safe and comfortable lifting, positioning and ambulation of patients in accordance with the principles of good body mechanics. Utilizes mechanical devises as necessary Performs simple applications of heat and cold. Provides care to the body of deceased patients in preparation for removal to the morgue. Provides support to the family/significant others. Secures property. Obtains, assembles and maintains equipment for use during diagnostic and therapeutic procedures. Assists members of the health care team with physical exam and other procedures. Conducts purposeful hourly rounding for assigned patients noting: Pain, Position, Proactive Toileting and Possessions Participates in the patient belonging process, including reporting of valuables to the RN and documenting accordingly. Transports patients by wheelchair, stretcher or bed, and escorts ambulatory patients. Performs errands away from the area of assignment to implement the care of patients Including transporting stat specimens and/or obtaining supplies as necessary. Maintains a clean uncluttered and tidy environment. Maintains equipment and cleans as necessary. Stocks supplies. Works in a safe manner and provides for the safety of patients and other employees. Utilizes aggression control measures in the care of patients who are verbally or physically threatening. Assists the RN with the application of restraints as required. Performs patient observations for Safety, constant/One to One and/or every 20 minutes checks of assigned observation status. Communicates information and basic instructions to patients, families, and visitors Supports the mission, vision, philosophy and goals of the Department of Nursing and The Mount Sinai Health System. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $85.9387 - $85.9387 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits. Responsibilities : Under the direction and supervision of an RN or LPN, performs various resident care activities and related non-professional duties for the specific needs and comfort of the aged and those that need our help. Assist residents with activities of daily living to include: bathing, dressing, grooming and toileting. Transfer residents to and from activities and meals according to their individual service plans. Serve meals to residents in the dining room or their apartments. Record and report changes in residents' eating habits to supervisor. Promote quality services within company, state and federal regulations. Qualifications : Current NY State Certified Nursing Assistant Education : High School Diploma/GED 
Responsibilities: The LPN is responsible for administration of all medications and treatments during their shift, according to the established medical and nursing care plan. Works under the direction of the Head/Charge Nurse to provide optimum nursing care to all residents on the unit. Continuously evaluates behavior, symptoms, reactions and progress of residents and discusses findings with Nurse Manager/Charge Nurse. Administers and documents all medications according to established policies and procedures, physician's orders, and Department of Health regulations. Performs all treatments according to physician’s specific order and established policies and procedures. Discusses the need to discontinue or change a medication of treatment with the Nurse Manager/ Charge Nurse. Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy.
