Orders and assists with coordination of EP patient admissions (changing meds, arrhythmia issues, etc) Assists with coordination of care between service lines when needed (referring out to other specialties) Assists with EP provider clinics, assisting patient and family education and instructions. Relays tests results (holter, MRI, EKG, Echo, etc) and necessary changes in care as requested by EP MD’s Collaborate with multidisciplinary teams to develop and implement individualized care plans. Coordinate outpatient visits and ensure continuity of care across settings. Maintain accurate and timely documentation in the electronic medical record.
