We’re hiring a Director of Patient Care Services – a compassionate and experienced clinical leader who oversees hospice care delivery, supports interdisciplinary teams, and ensures regulatory compliance. What You’ll Be Doing: Serve as a role model and resource for nursing staff and interdisciplinary team members Support staff in problem-solving and determining appropriate clinical interventions Collaborate with the Administrator on hiring, orientation, retention, and performance management Adhere to budget requirements for labor, pharmacy, DME, and medical supplies Coordinate patient care with RN Case Managers and attending physicians Review and manage patients receiving higher levels of care (respite, inpatient, continuous care) Oversee clinical and management responsibilities for assigned teams Serve as liaison between Administrator, clinical staff, patients, families, and community partners Qualifications: Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field. Current, unencumbered Registered Nurse (RN) license in the state of employment. Active CPR certification required; certification in Hospice and Palliative Nursing preferred. Minimum of 4 years of experience within the last 5 years in hospice, palliative care, supportive care, home health, or a primary care setting. Minimum of 1 year of supervisory experience in a healthcare setting. Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable. Valid driver’s license with a clean driving record. Reliable transportation and willingness to travel as needed. Ability to lead bereavement support groups and make sound clinical decisions. The position combines the activities of direct and indirect patient care and directs staff nurses, LVNs, CHHAs in the delivery of the individual patient’s plan of care as well as identifying interdisciplinary needs and coordination of the allied health clinicians. Be the Best RN Case Manager You Can Be If you meet these qualifications, we would love to meet you: Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation. A minimum of one year of experience as an RN. Current CPR Certification required. Responsibilities: As a RN Case Manager, you will: Assessment of the status of nursing needs of assigned patients and overall management of the patient’s care plan. Completes initial patient assessment upon admission including appropriate patient history. Selects appropriate nursing diagnosis. Assess all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications. Establishes realistic measurable, observable goals consistent with the patient’s diagnosis. Informs the physician of the results of the assessment and recommended clinical interventions. Implements appropriate nursing interventions consistent with the patient's diagnosis and established goals and within the scope of the Nurse Practice Act. Re-evaluates the patients nursing, and other clinical needs as required. Documents and submits the assessment, progress and discharge notes and other required paperwork in a timely fashion as required by the agency. Informs the physician and other personnel of changes in the patient's condition and needs.
