Hospice Admissions RN

Based out of our Temecual Office, The Admissions RN is a registered nurse and an Interdisciplinary Team member whose primary responsibility is to present ANCHOR Hospice services and philosophy to patients and families in a manner that results in the admission of appropriate patients. Participation in the information gathering, analysis, education and form completion aspects of the admissions process Evaluate patients for hospice appropriateness, e.g. medical records review as requested. Obtain agreement for admission of referred patients and families who meet admission criteria. Analyze data collected by the LPN to facilitate/obtain admission orders Build effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HOSPICE. Maintain clinical and professional competence. Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care. JOB REQUIREMENTS Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas. Ability to communicate effectively with patients and families, physicians, referral sources and colleagues. Knowledge of clinical manifestations of diseases and ability to develop ANCHOR specific clinical knowledge. Ability to use the ANCHOR information system and mobile EMR platforms. Reliable means of transportation. QUALIFICATIONS Currently licensed as a R.N. in the state where duties are performed. Maintains appropriate supply levels. Cost conscious in usage. Participates in and prepares for site inspections and inventories. Completes department inventories per guidelines. Enters authorizations and schedules surgeries and procedures in a timely and accurate manner. Assists in other departments as assigned to meet staffing needs and travels to other sites as business needs arise. Successfully completes Nursing Competencies with a score of 90% or greater on all but one section. Passed on retesting = Good. Unsuccessful in passing more than one section of Nursing Competency Testing and/or failed to pass testing on second attempt = Needs Improvement Quality initiatives Completes Quality Assurance (QA) assignments and ensures overall department compliance with requirements. QA scores: 96-100% = Great; 90-95% = Good; 90% and below = Needs Improvement. Maintains narcotic control procedures including key management and inventory. Assists with prescription reconciliation at the end of month. Support and completion of quality initiatives: P4P, BMI, DataMart, etc. Technical skills Demonstrates knowledge of equipment and Information Systems applications. Able to activate emergency procedure per protocol - code, fire, etc. Documents patient care events in a thorough and accurate manner. Manages and completes AEHR tasks per prescribed time lines. Support and knowledge of new applications and policies: AEHR, Abbreviations, etc. Demonstrate typing skills proficiency by: Using a keyboard, required to type proficiently and accurately; Have the ability to type a minimum of 30 words per minute with 0-2 errors; Have the ability to proof work.
