RN MDS Coordinator leads the MDS department, assists in coordinating discharge planning as well as assessing short term residents. The ideal candidate will possess skills to maximize reimbursement as well as ensure Medicare compliance. The RN MDS Coordinator may be responsible for managing short term managed care cases. The RN MDS Coordinator gathers information, assesses needs, establishes reasonable goals, provides interventions and incorporates within an organized, concise, functional care plan. Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident. Each plan must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified. Works together with care planning team to implement final plans. Encourages the resident and his/her “responsible parties” to participate in the development and review of care plans. Care plans must focus on assisting residents to reach their highest practicable level of well being. The RN MDS Coordinator ensures that all nursing personnel are aware of the care plan for each resident and that care plans are used in providing daily nursing services.
