Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients and more. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information. Performs other related duties as assigned. Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. Master’s degree in nursing or related field. 1+ year of experience in Nursing or relevant area. ACLS certification. Description Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transfer 2. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ens 3. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. 4. Inventories, orders or re-stocks medical supplies as applicable. 5. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. 6. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. 7. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. 8. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. 9. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. 10. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). 11. May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. 12. May be required to perform proficient phlebotomy duties. 13. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. 14. May maintain, adjust and confirm patient office schedule. 15. May check and verify & update insurance information utilizing internal or web sources. 16. May perform and verify pre-certification for procedures as necessary. 17. Performs other duties as assigned. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care. • Participates in the orientation of selected staff through ‘buddy system’ field observation of staff and feedback to appropriate staff involved in orientation process. • Participates in selected student programs by providing guidance, training and mentoring to students. • Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient/client care. • Transports and utilizes required medical equipment and other supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient/client homes/care facilities. • Travels to patient/clients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient/client. • Follows work procedures in relation to established workflows. • Maintains productivity sufficient to meet VNS Health goals. • Assumes responsibility for personal professional growth through attendance at in-services, training, continued formal education and select journal readership. • Performs other related duties and participates in special project, as assigned. • For CHHA and Hospice Only • Establishes, implements, and revises a plan of care with input from appropriate multidisciplinary health team members (i.e., physician, OT, PT, SW, SLP, family members, caregivers, HHAs, etc.) based on goals mutually acceptable to the patient and significant others. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Description The Assistant Nurse Manager of ID (Infectious Disease) Transplant / Ophthalmology / Ambulatory Float Pool is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Assists in the validation of staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis when Nurse Manager is away and as assigned 7. Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 8. Collaborates with the physician dyad and escalates concerns as appropriate. 9. Follows up on all patient concerns in a timely manner and provides service recovery as needed. 10. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Huddles with staff at the start of assigned shift for assigned unit(s) identifying high-risk patients who require special attention. 3. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Associate Director of Nursing/designee or Patient Relations as needed. 5. Supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans on assigned shift. 3. Collects data for unit(s) chart reviews, assessment of compliance with nurse sensitive indicators, HAIs and other patient safety goals metrics as assigned 4. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. 5. Participates in interdisciplinary quality improvement teams using the principles of high reliability. 6. Reviews unit progress, changes and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. 7. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. 8. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. 9. Leads staff participation in quality and safety initiatives and mandated compliance measures. 10. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration 1. Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. 2. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. 3. Works collaboratively with physician dyads and the healthcare team to promote patient care and regulatory standards. 4. Assists in the compliance of a nursing unit with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. 5. Oversees patient care delivery, throughput, staffing and processes to ensure efficient and effective operations on assigned shift. 6. Supports and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. 7. Maintains patient/employee confidentiality in the management of information. 8. Participates in the evaluation of nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. 9. Monitors availability and maintenance of nursing equipment and supplies. 10. Participates in regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. 11. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 12. Facilitates organizational change initiatives by using effective strategies. 13. Participates in a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. 14. Interprets and implements collective bargaining agreements affecting staff. 15. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. Professional Development 1. Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. This setting includes the management of patients in the Cardiothoracic ICU (CTICU), Surgical ICU (SICU), Medical ICU (MICU) and Intermediate Care Unit (IMCU). The APP is under the direct supervision of an in-hospital 24/7 Critical Care board certified Intensivist program. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in a specific ICU. Responsibilities: Provide direct clinical services for critically ill adult patients with multi-system disease /failure, according to established protocols and evidence-based clinical practice standards. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Manages continuity of care to all Hospice patients and their families after normal operating hours. Responds to incoming calls from patients/families/caregivers, assesses and priorities patient needs based on policies and procedures and staff capacity, ensuring all emergent and urgent request are met timely. Addresses needs within context of patient's Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. • Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. • Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. • Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. • Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. • Participates in special projects and performs other duties as assigned.
