Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations You will report to the Clinical Team Manager. The base salary for this position is $45.00 per point based on the company’s good faith estimate at the time of posting. Registered Nurse (RN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations Provides and receives nursing reports upon reporting in and ending shift duty hours. Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner. Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. Prepares and administers medications and performs treatments as ordered by the physician. Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Counts all controlled drugs with other nurse at shift change and notifies management of all drug and narcotic discrepancies noted on your shift. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Admits, transfers and/or discharges residents as necessary to meet the resident’s individual needs and maintains the overall safety of other residents, staff and visitors. All other duties, as assigned. Registered Nurse (RN) Skills and Abilities: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Ability to organize and prioritize work. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Registered Nurse (RN) Education and Experience: Current CPR Certification required. Nursing Degree or diploma from an accredited College or University. Possess a current license to practice in the State as a Registered Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required.
