Follow resident service plans and individualized care instructions. Answer call lights promptly and provide compassionate support to residents. Assist with meal service, hydration, escorts to meals/activities, and resident engagement. Encourage resident independence, dignity, and socialization. Promote Safety & Teamwork Report resident changes, incidents, injuries, or safety concerns immediately to leadership or nursing staff. Maintain a clean, organized, and safe work environment. Follow infection control, OSHA, and safety procedures. Assist with admissions, transfers, and discharges as assigned. Participate in staff meetings, trainings, and continuing education opportunities. Support teamwork by maintaining positive communication and professionalism with coworkers, residents, and families. What You Bring to the Team Credentials & Experience Medication Aide/Medication Administration certification required (per state regulations). Caregiver or CNA experience preferred. Previous experience in assisted living, memory care, or long-term care preferred. CPR certification preferred. Valid driver’s license preferred (where applicable). We provide specialized care to patients recovering from stroke, orthopedic procedures, neurological conditions and other life altering trauma. You would be joining a collaborative unit that includes a team of Physical Therapists, Occupational Therapists, Speech Therapists and Physiatrists. This role focuses on providing high quality, patient-centered care. We are dedicated to helping patients regain independence and improve quality of life.
