JOB SUMMARY Promotes and restores patients' health by completing the nursing process; collaborating with physicians and intermultidisciplinary team members; providing physical and psychological support to patients, friends, and families. The graduate nurse who holds a TPP must practice under the supervision of an experienced, Pennsylvania registered nurse who is physically present in the unit or area where the graduate nurse is practicing. ASSURES QUALITY OF CARE By adhering to therapeutic standards measuring health outcomes against patient care goals and standards making or recommending necessary adjustments following hospital and nursing division's philosophies and standards of care set by state board of nursing, State Nurse Practice Act, and other governing agency regulations by participating in performance improvement activities by assessing care provided to assigned patients by LPNs, Nursing Assistants, agency personnel, and other associates. RESOLVES PATIENT PROBLEMS AND NEEDS By utilizing multidisciplinary team strategies by utilizing established communication methods to ensure resolution of patient problems and needs by communicating patient status to the interdisciplinary team. DOCUMENTS PATIENT CARE SERVICES By documenting delivery of services, changes in the patient’s condition and the patient’s response to nursing interventions in the medical record according to policies and procedures. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. ESSENTIAL FUNCTIONS PROVIDES PATIENT CARE By prioritizing, planning and completing patient care as assigned attending to personal care needs assisting with patient transfers and ambulation obtaining accurate vital signs and other clinical parameters and and specimens including: blood sugar readings, EKG’s and drawn blood samples responding to patient requests requesting RN or LPN supervision with new or unfamiliar tasks. COMMUNICATES INFORMATION By collecting and documenting in the patient record providing clinical reports at regular intervals to fellow nursing assistants and nurses notifying RNs of significant findings or change in patient condition. DOCUMENTS RESIDENT/PATIENT CARE SERVICES By recording information in the chart as soon as possible after collection recording intake/output totals at the end of shift or specified times records patient activity and care on the chart. MAINTAINS PATIENT CONFIDENCE AND ORGANIZATIONAL CONFIDENTIALITY By keeping information confidential responding to patient requests using Tact and diplomacy. COLLECTS RESIDENT/PATIENT INFORMATION By operating patient care equipment, including blood glucose monitor, blood pressure cuffs, lifts and scales according to policy and procedure observing patient response to care engaging patient in conversation and acting upon responses or requests. PROVIDES A SAFE AND CLEAN WORKING ENVIRONMENT By completing environmental rounds keeping patient rooms, supply rooms and hallways free from clutter applying infection control policies and procedures to daily practice and other unit specific equipment checks and maintenance duties meeting dress code requirements and maintaining adequate personal hygiene. SUPPORTS UNIT EFFICIENCY By cleaning and restocking patient equipment assisting with cleaning and monitoring of equipment contribute to PI activities using equipment and supplies as needed to accomplish job results reporting maintenance and equipment issues per policy reporting to assigned unit on time, ready to meet the responsibilities of the position and Punching in and out as per Good Shepherd policy and maintaining Etime sheet as necessary. Entity: Penn Medicine Doylestown Health Department: Inpatient GI Lab Location: 595 West State Street, Doylestown Pa 18901 Hours: Full Time Days Job Summary: To provide professional nursing care in accordance with the Nurse Practice Act, to collaborate with the physician's medical plan, and to execute the objectives and follow the policies of the hospital. To assist the surgeon in all procedures including advanced procedures in both the GI suite and the operating room. Accountabilities : • 1. Assesses, plans, implements, documents, coordinates, and manages patient care according to department and hospital standards and giving consideration to age-specific accommodations. • Utilizes the nursing process for all patients assigned to his/her care • Implements nursing/physician orders that result in achievement of organizational, nursing and physician goals. • Utilizes analytical skills to provide efficient patient care and be able to handle critical situations in a timely and safe manner. • Able to provide care to the critical of patients including those requiring airway and/or ventilatory support and hemodynamic monitoring and intervention. • Develops, implements and documents care plan based on nursing/physician orders, evidence-based practice and according to department standards. • Demonstrates a comprehensive knowledge of normal physiology and disease • pathophysiology, appropriate nursing and medical interventions and a knowledge of the principles of growth and development over the lifespan. • Demonstrates appropriate skill in utilization of the EMR. • Documents assessments, patient treatment, response and effectiveness of care according to department guidelines. • Provides teaching and discharge planning based on assessment of self-care knowledge/ability on an ongoing basis. • Ensures appropriate completion of chart and other documentation including transcription of physician orders, charge entry and medication reconciliation. • Appropriately delegates all Associates under his/her supervision according to their skill level. • Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs. • Performs selected therapeutic interventions and diagnostic procedures according to department standards as ordered to include but not limited to: ◦ medication administration according to hospital policy ◦ point of care testing ◦ cardiac and hemodynamic monitoring ◦ initiation and maintenance of oxygen therapy ◦ specimen collection utilizing mobile lab when appropriate ◦ phlebotomy (venipuncture) ◦ Adheres to Moderate Sedation Guidelines pre, intra and post procedurally. ◦ Thoroughly completes the GI Monitoring Record for all sedation cases. ◦ Successfully performs and records a “Time Out” before each procedure. ◦ GI Chart checks to be done daily, the day prior to the scheduled procedure. • Assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention. • Demonstrates ability to perform with clinical proficiency in crises and medical emergencies. • Demonstrates ability to be technically competent in all psychomotor interventions when administering physical and psychosocial care to patients. • Implements decontamination, cleaning and reprocessing of endoscopic equipment and accessories according to policy and procedure. Tests proper functioning of instruments. • Prioritizes activities based on knowledge and comprehension of the schedule. • Demonstrates knowledge and technical skills in use and care of endoscopic equipment and accessories. • Assists the surgeon during all procedures including but not limited to EUS, EBUS, Diagnostic bronchoscopy, colon/rectal ultrasound, ERCP. • Assists with administration of genetic testing in conjunction with the procedure. • Demonstrates critical decision-making skills while providing care by analyzing data and modifying approach based on clinical status. Will consult with appropriate personnel and take appropriate action in a timely manner. • Assists with specimen collection and proper labeling, preparation and transport. • Assists with Ordering Supplies and checking expiration dates. • 2. When appropriate, adheres to defined computer generated quality monitoring or procedures for validation of equipment functionality. • Safely administers medications and treatments as ordered according to hospital policy and formulary using the appropriate principles of safe medication administration, 2 patient identifiers. • Safely handles various types of equipment, drugs and body fluids using knowledge of appropriate standard and cleaning precautions. • Participates in the surgical “time out” prior to procedure. • Follows staffing and on call guidelines. In the interest of patient care, responds to changes or needs within the Patient Services Team by cooperating with staff rotation, reassignment to other departments and weekend/holiday scheduling. • 4. Demonstrates and maintains competency in the care of patients during all procedures including proper handling, use and cleaning of all equipment • Demonstrates and maintains technological competency in skills required for participation in advanced procedures such as Endoscopic Ultrasound (EUS), Endoscopic Bronchial Ultrasound (EBUS), Diagnostic Bronchoscopy, Colon/Rectal Ultrasound, Endoscopic Retrograde Cholangial-pancreatography (ERCP), genetic testing, and GI procedures.
