LPN Med Nurse (7:00 AM – 3:00 PM) Full-Time & Part-Time Available | Union Position Berkshire Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. As an LPN Med Nurse, you will play a vital role in delivering exceptional care by administering medications and treatments, monitoring resident health, and collaborating with our interdisciplinary team to ensure the highest quality of care. Schedule We are currently hiring for two (2) positions on the 7:00 AM – 3:00 PM shift : One Full-Time position One Part-Time position These are union positions , and every other weekend is required . Qualifications Current New York State LPN license in good standing. Previous long-term care or skilled nursing experience preferred. Strong communication and organizational skills. Ability to recognize and report changes in resident condition. Experience with electronic medical records (EMR) is a plus. Compassionate, dependable, and team-oriented. Responsibilities Administer medications and treatments in accordance with physician orders and nursing standards. Document medications, treatments, and resident care accurately in the EMR. Monitor residents for changes in condition and communicate findings to the nursing team. Perform nursing procedures including wound care, blood glucose monitoring, tube feedings, and other assigned treatments. Maintain medication carts and medication storage areas in compliance with regulatory standards. Ensure accurate controlled substance counts and follow all medication safety protocols. Collaborate with physicians, nurse practitioners, therapists, CNAs, and other members of the healthcare team. Follow infection prevention, resident safety, and facility policies. Perform other nursing duties as assigned by the Nursing Supervisor or Director of Nursing. This is an excellent opportunity for an experienced NP or motivated new graduate who has a strong interest in endocrinology and is looking to develop specialized expertise in the management of diabetes and other endocrine conditions. The Nurse Practitioner will work closely with Dr. Md Shajjad Hossain and the broader multidisciplinary team, with progressive autonomy as clinical knowledge and experience within endocrinology develop. About the Role The Nurse Practitioner will provide comprehensive care to patients with a variety of endocrine conditions, with a significant focus on diabetes management and longitudinal follow-up. The position offers the opportunity to build specialized endocrine expertise while working collaboratively with experienced physicians and APPs. The NP will initially work closely with the physician team on more complex cases while progressively developing greater independence with routine endocrine follow-up care. The practice averages approximately 25–30 patient visits per day. Collaborating Physician – Dr. Md Shajjad Hossain The Nurse Practitioner will work closely with Dr. Md Shajjad Hossain, who will serve as the collaborating physician for this position. Dr. Hossain is committed to developing an APP who is interested in building expertise in endocrinology. Develop expertise in the evaluation and management of diabetes and other endocrine conditions. Review continuous glucose monitoring (CGM) data and make appropriate treatment recommendations. Adjust diabetes medications and insulin regimens based on clinical findings and patient needs. Support patients utilizing diabetes technology and provide ongoing education regarding disease management. Order, review and follow up on laboratory and diagnostic testing. Collaborate with physicians on complex endocrine cases and recognize when physician consultation is appropriate. Share responsibility for the clinical in-basket, including patient messages, test results, medication refills and follow-up needs. Provide clinic and hospital cross-coverage as needed. Educate patients regarding medications, diabetes technology, lifestyle modification and ongoing disease management. Maintain accurate and timely clinical documentation. Qualifications Graduate of an accredited Nurse Practitioner program. Current or eligible New York State Nurse Practitioner license. Appropriate national board certification. BLS/ACLS/PALS required.
