Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 11pm-7:30am, Sat & Sun. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of professional nursing care in Telemetry. The Registered Nurse is directly responsible and accountable for the care given to his/her patients in the Emergency Department. They communicate with the physician about the changes in the patient’s clinical condition including results of the diagnostic studies and symptomatology. Is capable of responding quickly and accurately to changes in the condition and/or response to treatment. Is knowledgeable about EMTALA regulations and reporting of child/adult abuse to appropriate agencies and Hospital Social Worker. Maintains patient privacy and confidentiality. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Current ACLS certificate 30 days upon hire and maintain current. 4. Current PALS certificate and/or ENPC 30 days upon hire and maintain current. 5. Basic Arrythmia Interpretation within 30 days of hire. 6. Minimum of one year experience as a staff nurse RN in an acute care hospital setting; E.R. setting preferred.7. Certified Emergency Nurse (CEN) preferred.8. Bachelor of Science in Nursing (BSN) preferred. Facility Specific Requirements (facility may require items listed below): 1. In Paramedic Base Station ED: MICN within 12 months of hire and maintain current. 2. EDAP certified facilities: Completion of emergency pediatric course 1 and 2 within 6 months of hire.3. Non-OB facilities: NRP certificate 90 days upon hire and maintain current.4. Trauma Nurse Core Course (TNNC) within 12 months of hire required.
