What You’ll Be Doing Develop detailed Plans of Care for assigned patients, ensuring all services are identified and met Collaborate with the Interdisciplinary Group, patient, family, and attending physician to coordinate care Provide skilled hospice and palliative care for the remainder of the patient’s life Conduct comprehensive nursing evaluations and assessments Provide field supervision to LVNs and Home Health Aides assigned to your patients Perform field case management and other duties assigned Qualifications Registered Nurse with a valid state license Minimum of 1 year experience as an RN required; hospice experience preferred Ability to lift up to 50 pounds Must clear criminal background check, physical, and drug screening Valid driver’s license, good driving record, and reliable transportation required Willingness to drive to patient locations and work outside regular hours as needed Why Choose Silverado? Instructor - Physician Assistant or Nurse Practitioner

Job Purpose Provides advanced patient care to patients in a clinical setting in an area of specialization under the direction of a physician. Job Duties Obtain detailed history and perform physical examinations. Perform, order, and interpret diagnostic procedures in accordance with relevant health concerns of the patient. Prescribes appropriate pharmacological and non-pharmacological treatments. Maintain records and write orders and progress notes in patient charts as to status, treatments, and procedures. Monitors and ensures accuracy of recording on patient medical records. Provide follow-up health maintenance care to patients in accordance with protocols established with the supervising physician. Communicate with physicians, patients, and the family members of patients concerning patient care. Collaborates with providers, and practice staff in identifying appropriate patients for care management. Perform other job related duties as assigned. Office-Based Urologic Procedures and Clinical Responsibilities: Perform flexible cystoscopy and related office-based urologic procedures within scope of practice and departmental protocols. Perform catheter insertions, exchanges, and management of difficult catheter placements. Conduct voiding trials, manage urinary retention, and perform diagnostic evaluations including uroflowmetry and post-void residual measurements. Assist with and perform additional urologic procedures as approved by departmental leadership and supervising physicians. Evaluate procedural outcomes and provide appropriate post-procedure management and follow-up care. Additional Clinical Duties: Conduct independent urology clinic visits, including new patient evaluations and follow-up appointments. Participate in prostate cancer surveillance and treatment pathways. Support management of procedural clinics, including Artemis fusion biopsies, cystoscopy clinics, and other specialty services. Assist with provider coverage and patient access initiatives across the Department of Urology. Perform additional clinical and procedural duties consistent with licensure, training, competency validation, and departmental needs. Minimum Qualifications For Nurse Practitioners: Master’s degree in Nursing. Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Current Texas Licensure as an Advanced Practice Nurse (APRN) For Physician Assistant: Master’s degree in physician assistant studies. NOTE: Bachelor’s degree in physician assistant studies accepted based on terminal degree requirements at time of graduation. Experience Required: none required. Certification/Licenses/Registration: For Physician Assistant: Current license as a Physician Assistant by the State of Texas Physician Assistant Board. Specialty certification as required by department. For Nurse Practitioners: Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
