Reviews with the SWA to ensure that discharge planning page in CarePort, is current and accurate for reporting purposes. Monitors length of stay, timely scheduling of tests/consultations and reports barriers to department leadership for intervention. Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Newark Center City office for the Child Health Program within the School of Nursing. The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.
