Nurse Case Manager

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP). Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. Description Patient Care Associate MSH GP9C Med/Surg FT Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Position Summary The Registered Nurse (RN) provides on-site nursing care under the supervision of the Nurse Manager. This role focuses on supporting clients’ medical needs, coordinating care, and promoting overall health and stability. Qualifications Active New York State RN license. Experience working with homeless or vulnerable populations is a plus. Position: Per-diem Hours: Flexible Hours, Evenings & Weekends Start Date: Immediately As a Home Care PRN Infusion Registered Nurse (RN) , you will experience a special energy and excitement from your work as you join a team of individuals who are committed to improving the lives of others. You will work in a growing, specialized area of the nursing field outside the walls of a hospital setting As a Home Infusion Registered Nurse you will provide a broad range of infusion therapies in out patients’ homes while: Compassionately taking care of patients in order to allow them to recover in the comfort of their own home by using your clinical IV capabilities and personal touch. Skillfully providing one-on-one home infusion nursing care visits and patient education in accordance with the physician’s plan of treatment. Assessing the patient’s condition and response to therapy, taking action as needed to facilitate achievement of treatment goals. Establishing trust and building therapeutically effective relationships with our clients and their families.
