163159 Job Description Serves as liaison between the patient and facility/physician. Ensures a continuum of quality patient care throughout hospitalization and oversees provisions for patient's discharge. Assesses, plans, oversees and evaluates the appropriateness of care throughout admission and hospitalization of the patient. Job Responsibility Facilitates patient management throughout hospitalization. Performs concurrent utilization management using evidence based medical necessity criteria. Submits data to management regarding case management and/or quality initiatives. Serves as an in-patient liaison - planning, assessing, implementing and evaluating patient in collaboration with the health care team. Conducts chart review for appropriateness of admission and continued length of stay; Facilitates throughput. Conducts clinical reviews and interacts with third party payers to obtain approval of hospital days, pre-certification and post-discharge eligibility in relation to clinical course. Ensures compliance with current state, federal, and third party payer regulations. Documents the case management process in the medical record; completes and documents a care coordination assessment on the patient. Serves as a liaison to patient, family, admitting, primary care physician, health care team, and hospital departments. Completes applicable areas of the patients discharge instruction sheet and the patient transfer sheet; ensures case management documentation is current and accurate. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. Duties Description Are you a Registered Nurse with experience in nurse education and training? Kingsboro Psychiatric Center is currently recruiting a Registered Nurse 3 (Psychiatric Educator) to plan, design, and develop training curriculum to improve the knowledge, skills, and abilities of nursing staff. As a Registered Nurse 3 (Psychiatric Educator), you would: Plan, design, and develop orientation and training curriculum for nursing staff; Provide on the job training Clinically supervise staff in the provision of nursing services Evaluate training needs and modify programs to ensure effective service delivery Collaborate with nursing management and multidisciplinary treatment teams to develop and revise policies and procedures and ensure staff competencies are current and in compliance with nursing standards Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they are: Licensed and currently registered as a registered professional nurse in New York State; and Bachelor’s degree in nursing; and 2 years of post-licensure nursing experience. One year must have been clinical nursing experience working with individuals, at least 50% of whom have been diagnosed with mental illness, addiction, or substance use disorder; and 1 year must have been professional nurse educator experience conducting nurse education and training. Substitution: A master’s degree in nursing may substitute for 1 year of post-licensure clinical nursing experience A doctorate in nursing may substitute for 2 years of post-licensure clinical nursing experience. OR Candidates within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
