Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. This position offers unique experience in our community pediatric intensive care unit, for an individual who wishes to pursue a career in pediatric intensive care. The section of Critical Care Medicine in the Department of Pediatrics is a multidisciplinary team of attending critical care physicians, advanced practice providers (nurse practitioners/physician assistants), committed to the care of patients suffering from a life-threatening illness or injury. Currently, the critical care intensive care units in the community have capacity for 14 patients. The NP/PA Instructor will work alongside Faculty colleagues, advanced practice providers, and occasionally pediatric residents in the care of children with chronic and acute illness requiring intensive care in the PICU. The woodlands PICU opened in April 2017 and provides assisted ventilation or other advanced support, such as oscillatory ventilation, nitric oxide administration or cardiovascular pressor support. The community ICU also provides care for children who require long-term technology support, those with residual PICU morbidities requiring transition to home, and the management of children who require more intensive medical support. Location: The primary clinical site is at Texas Children's Woodlands Campus, an extension of the largest pediatric hospital in the United States, and an affiliated hospital of Baylor College of Medicine. Role responsibilities include but are not limited to: Day-to-day care of children with acute and chronic illness requiring critical care. Active role in education of trainees when present. Participation in on-call schedule for the pediatric intensive care unit. Liaison and collaboration with stakeholders and consultants throughout the community hospital. Position Summary Responsible for maintaining the standards of nursing care and practices within the facility to meet the physical, psychosocial, and rehabilitative needs of the residents Essential Duties and Responsibilities · Administers enteral feedings along with oral, injectable, and intravenous medications and treatments within the LVN scope of practice as prescribed by the physician and per facility policy · Ensures all aspects of delivery of care to residents by nursing assistants are completed within the established time frames according to Resident Care policies and procedures · Performs and is co-responsible for narcotic counts as per Resident Care policies and procedures · Ensures dispensing of medications and treatments by all routes as ordered by the physician within established time frames. Ensures no medications are left unattended · Documents medication and treatment administration in a timely and accurate manner · Completes/updates the Electronic documentation as required · Maintains and updates Nursing license in compliance with state regulations · Performs other related duties as required or requested Minimum Qualifications (Knowledge, Skills, and Abilities) · College degree or the equivalent in work related experience · Must be graduate of an accredited school of Licensed Vocational Nursing · Must have active state LVN license · Must be listed in good standing as confirmed on Nurse Registry · Strong knowledge of state, federal, and local regulations as they pertain to long term care is strongly preferred · Must be able to communicate verbally and in writing in English Physical Demands and Work Environment The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Job Summary The Clinical Nurse Care Manager (CNCM I) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I facilitates transitional care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. In addition, the CNCM I helps to drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, process improvements, patient satisfaction). CNCM I is accountable for patient's caseload and plans effectively in order to meet patient needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote efficient utilization of resources. CNCM I advocates for patients and coordinates patient's care and work with other health care professionals to determine patient's goals and create both short term and long term plans. CNCM I participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members. Minimum Qualifications Degrees: Graduation from an accredited school of Nursing with a Bachelor's degree required. License & Certifications: Register Nurse; Licensed to practice nursing in the State of Texas. Related specialty certification preferred Basic Life Support Work Experience: Three (3) years experience as RN in appropriate area assigned required; One (1) year experience in Case Management, Quality Management, and Coding preferred. Management Experience: One (1) year leadership/clinical experience preferred.
