Organization: VWCH Float Pool Job Summary: JOB SUMMARY Responsible for assessing, implementing, and evaluating care for various patients within the facility. Reports directly to the Clinical Director of the unit. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position. (7p-7a) Hours are: 7pm-7am -Premium pay is $3 for Non-CC and $5 for CC areas -Shift Differentials are: $3 for 7p-10:30p; $3.50 for 10:30p-7a and an additional $6 for weekend hours -Experience required: A minimum of 2 years of ICU or ED experience . Demonstrates sufficient acumen to recognize problems, ask questions and to contact the patient's primary care provider and other members of the healthcare team. Has experience in gathering pertinent clinical data that aid in referral, treatment, or other primary care pathways. • Nursing Patient Education (Novice): Possesses sufficient fundamental proficiency in providing patient education in practical applications of moderate difficulty. • Care Planning (Novice): Demonstrates ability to anticipate resource needs, identify distinct tasks, set priorities, schedule activities, meet deadlines, and organize work within areas of direct responsibility. Responsibilities include: Performing patient assessments, H&Ps, daily progress notes, and ongoing management of hospitalized burn patients. Providing patient and family education, communicating treatment plans, and serving as a key liaison for families throughout the hospitalization. Coordinating care with multiple disciplines, including surgery, nursing, rehabilitation services, case management, social work, nutrition, and other consulting teams. Performing burn dressing changes at the bedside and assisting with dressing changes completed under sedation. Facilitating transitions of care and coordinating discharge planning and outpatient follow-up. Participating in multidisciplinary rounds and collaborative care planning. In addition to clinical responsibilities, the position may include: Updating and maintaining burn service protocols, clinical practice guidelines (CPGs), and workflow processes.
