Responsibilities: Under direction of the RN or LPN reinforces educational needs to adolescent, adult, and geriatric patients who present with readiness to learn. Assists with dietary trays and feeds patients when indicated. Turns, repositions, and assists in ambulation of patients as ordered using proper body mechanics. Keeps accurate records of daily patient activities and records the information as necessary. Performs post-mortem care on patients following nursing procedures. Knowledgeable in the growth and developmental needs of the middle adolescent through the geriatric patient. Takes and records vital signs accurately. The employee exhibits the ability to accurately assess and adjust interventions based upon patient age. Assists with dietary needs monitoring patient intake when necessary through I&O or titration of fluids. Reports and/or records any changes observed in condition or behavior of patient. Qualifications: Work requires the ability to read well enough to follow specific and detailed nursing procedures, the ability to write legibly, use good English grammar, spell accurately and communicate effectively with a variety of people. This level of education is normally acquired through completion of a high school education. Completion of a certified nursing assistant program or previous experience. Job Summary The Registered Nurse, Emergency Department is responsible for handling initial patient assessments, performing laboratory work, point of care testing and administrative tasks associated with the delivery of patient care. The Emergency Department Registered Nurse collaborates with physicians regarding discharge plans, creates and educates patients upon discharge from the Emergency Department and works effectively in transferring patients who are admitted to the hospital. Job Responsibilities Triages patients who present to the Emergency Department, prioritizing care based on the critical nature and severity of a patient’s condition. Assesses patient condition through visual evaluation, medication reconciliation, and vital sign measurements such as: blood pressure reading pulse rate, respiration, and temperature throughout the course of a patient’s stay in the Emergency Department. Administers medication and assists with medical procedures as prescribed by the treating physician. Monitors patients for changes in condition and communicates timely and effectively with those involved in the welfare, care or treatment of the patient in compliance with applicable policies and regulations. Partners with other members of the patient care treatment team in the provision of procedures, interventions, discharge or admission processes. Charts all patient medical history, contact information, condition, medications, and treatment as well as update the electronic medical record throughout the patient stay in the Emergency Department. Job Summary The Perianesthesia Nurse is a registered nurse responsible for the care of patients being prepared to undergo, or recovering from, anesthesia. The Perianesthesia Nurse is tasked with the medical management of each patient and has the skill and knowledge to address critical medical emergencies pre- and post-surgery. Job Responsibilities Prepare patient for surgery or procedure. Ensure the patient chart is initiated, complete the patient assessment, confirm the procedure and/or surgical site, initiate patient IV, perform ordered labs, dispense ordered preop medications and fully document all information and care provided. Provide instructions to patient and support persons as to what to expect throughout the surgery process. Coordinate patient care needs with other disciplines to ensure continuity of care. Monitor patient recovery from anesthesia and medical procedures. Regularly monitor and record patient vital signs, including heart rate, blood pressure, temperature, and respiration rate. Constantly assess patient for adverse reactions to the anesthesia and respond appropriately. Educate each patient on the potential for reactions or side effects of anesthesia. Provide post-procedure discharge instructions to the patient and authorized patient representative that include physical, psychological, social, safety and educational and needs of the patient. Provide orientation and training of new employees as assigned. Function as testing personnel under CLIA1988 regulations as they relate to point of care testing as required. Exceptional Skills And Qualifications Marketing Liaison: Monitor along with marketing lead, the conversion rates of hospital referrals and problem solve issues to work on increasing conversion rates to at least 60%. Marketing rounds to include thank you notes/treats, updates on any new hospice changes or programing, feedback on cases, doing a needs assessment on any improvements in flow or communication of discharges. Clinical Liaison will make marketing rounds weekly to ensure Palliative team, Case managers and all referral sources know we are actively seeking new referrals. Work with marketing lead to visit top referring physician offices in the immediate area of the hospital to allow the staff to have a “face with a name”. This will increase our visibility and create rapport with the physicians, navigators and staff of our network partners. Collaboration: Meet with Palliative Care and Case Management team on daily basis to assess their patient needs to discharge home on hospice care. Work in coordination with Palliative Care and Case Management to communicate all discharge plans, family dynamics and our admission times once discharged, and any other pertinent details that will help with seamless discharge for the hospital teams. Ensure hospice admission nurse is aware of all details of the case. Be on Palliative Care call every morning to report and give updates on current palliative cases that they have referred to Community Hospice. Hospice/Referrals: Enable all hospice patients to have a safe discharge to their home by ordering all equipment in a timely manner, making sure they have the proper medications, ambulance transport is set and all other items that might be needed for each particular case. Follow patient home and conduct same day admission. Allowing for continuum of care if the patient needs same day admission and our admission team is not able to do the admission. Admit any GIP’s onto our service in the hospital that you are assigned to, making daily rounds and coordinating care with our IDG team and NP’s. Create a plan for discharge with hospice staff, if patient goes home. Educate all referral sources on hospice appropriate patients and need for early referrals so we can plan for same day admissions as we are already involved in the case. Report to the hospital immediately after the Palliative Care call to follow up on any new referrals and check in with all referral sources. Experience Requirements Minimum: None Preferred: One (1) or more years of Medical Office experience, previous patient care experience in the physician office specialty area. License/Certification Requirements Minimum: Certified or Registered Medical Assistant and Basic Life Support (American Heart Association)
