Director of Nursing (RN) | Skilled Nursing Facility Evergreen Commons Rehabilitation & Nursing Center is seeking an experienced, motivated, and resident-focused Director of Nursing (DON) to lead our nursing team. As the clinical leader of our 242-bed skilled nursing facility , you will oversee nursing operations for both short-term rehabilitation and long-term care while fostering a culture of excellence, teamwork, and compassionate care. Maintain survey readiness and support successful Department of Health surveys. Mentor, coach, and develop nursing leaders and staff while promoting accountability and professional growth. Collaborate with the interdisciplinary team to ensure exceptional resident outcomes. Oversee and partner with two Assistant Directors of Nursing (ADONs) to support daily clinical operations. Promote a positive, resident-centered culture focused on quality care and customer service. Qualifications Current New York State Registered Nurse (RN) license. Graduate of an accredited school of nursing; BSN preferred. Minimum of three (3) years of experience in geriatric or long-term care nursing. At least two (2) years of progressive nursing leadership or nursing administration experience. Experience with New York State Department of Health surveys and regulatory compliance preferred. Strong leadership, communication, organizational, and problem-solving skills. Duties Description The RN 1 serves as a member of the interdisciplinary team performing various nursing duties, including coordinating and monitoring all aspects of medical care for residents. Duties And Responsibilities Include But Are Not Limited To Provide direct nursing care to residents, including comprehensive nursing assessments, administering medications, implementing physician’s treatment orders, transcribing orders, and collecting specimens for laboratory analysis or culture. Provide health education to residents, their guardians and staff and participate in interdisciplinary support plan meetings. Represent nursing on the resident’s support team, provide information on nursing needs, consult and coordinate with support teams and other health care disciplines to assess, plan, implement, or evaluate resident’s care. Provide first aid and emergency nursing care. Assess, monitor and document resident’s symptoms, conditions, and care, including diet and physical activity. Identify and report resident’s responses to medications or changes in health status. Monitor resident’s medication, ensuring medications are ordered and available for administration. Meet documentation standards regarding transcribing physician’s orders, nursing progress notes, health charts and support plans. Acts as a liaison to community providers to ensure continuity of care, follow up with community providers, when necessary, advocate for resident’s needs.
