Notifies a supervisor at once if noting any adverse reaction of medication on resident or awareness of medication error. Observes and communicates to the Wellness Coordinator, Nurse/Executive Director any changes in resident physical or mental status. Assists with ADLs and assignment logs for residents whether they are on the assignment sheet or not and reporting any significant changes in residents or declines.
