Purpose Statement/Position Summary: The Registered Nurse for Neurology, under general supervision, conducts patient assessments, participates in the development of the plan of care, implements and evaluates care according to patient specific age and/or developmental needs. Collaborates with other health care team members in the delivery of care. Employee will follow applicable policies and procedures in handling supplies, pharmaceuticals, and equipment. The Neurological Institute serves patients with a wide range of neurological diagnoses and serves hundreds of patients a week in our 34-patient room clinic space. Broad knowledge of patient care principles and equipment; CHLA patient care policies and procedures. Perform medication reconciliation, identify discrepancies or concerns, and communicate findings to the provider. Provide patient education regarding medications, treatment plans, preventive care, immunizations, disease management, lifestyle modifications, and follow-up care. Perform routine clinical screenings and preventive assessments, including blood pressure, pulse oximetry, BMI, vision screening, and other age-appropriate screenings. Obtain and review patient consents and ensure required documentation is complete and accurate. Coordinate patient care through referrals, appointment scheduling, orders, laboratory services, and follow-up activities. Collect, label, and prepare specimens for laboratory testing in accordance with proper handling and transport procedures. Monitor patients for adverse reactions to medications, immunizations, and treatments and promptly report concerns to the provider. Support preventive health and wellness initiatives, including immunization review and patient education. Maintain clean, organized, and properly stocked exam rooms, equipment, supplies, and emergency preparedness materials in accordance with safety and infection control standards. Support efficient patient flow and front- and back-office operations, including discharge, phone calls, scheduling, and referrals. Participate in quality improvement, productivity, compliance, and documentation initiatives and maintain compliance with HIPAA and applicable healthcare regulations. On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life! Responsibilities The Clinical Supervisor supports administrative responsibility for the department service functions and works collaboratively with the unit Director and/or Manager. The Clinical Supervisor prepares, conducts and/or oversees department(s) orientation and training of personnel. Communicates with staff, physicians, and administration both written and verbally. The Clinical Supervisor is responsibility and accountable for the quality of staff and functions during their shift. Assists the Department Director in the daily operations of the unit, directing, supervising, and evaluating personnel, ensuring the proper use of time, equipment and staff. Ensure the efficient functions of the department. Performs direct patient care, only as needed, within the scope of practice. Continually assess care and direct staff to make immediate changes in the plan of care. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid California Registered Nurse license2.
