The Admission RN is expected to complete a minimum of two admissions per day (or the equivalent of six visit points). Based on patient and agency needs, the nurse may also be required to perform follow-up visits. Additional compensation is provided for visits exceeding 24 visit points in a 32-hour workweek or 30 visit points in a 40-hour workweek, per the agency’s standard per-visit rates. This role reports directly to the Homecare Clinical Manager. Home Health Admission RN Job Responsibilities: Provide skilled nursing care, including patient admission and discharge, in a home-based, family-centered environment while maintaining complete and accurate patient records in electronic and/or paper formats. Educate, demonstrate, supervise, and assess care provided by peers, Home Health Aides (HHAs), family members, and caregivers, fostering a therapeutic relationship that promotes self-advocacy and independence. Ensure timely and accurate documentation of patient information, including OASIS and HIS data sets, medication reconciliation, service orders, time sheets, and other required patient and employment records.
