Description Patient Care Associate MSH GP9C Med/Surg FT Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Notifies RN if rounding has not been completed. • Utilizes Vocera system/Mount Sinai phone according to sign in/sign out protocol where activated. • Performs other duties as assigned, including generating reports • Is open and responsive to the diverse backgrounds and experience of other people and promotes an environment that is sensitive to cultural diversity. • Respects and values each patient’s unique, individual needs, beliefs, and diversity and incorporates these into delivery of care. • Greets visitors and answers telephones promptly and courteously. Escorts visitors to appropriate area, when necessary. Provides information as appropriate. • Answers Patient Call System and refers calls to appropriate staff. • Maintains a safe, clean and functional environment, including removing trash and bringing physical plant and equipment problems to the attention of the managers. • Transports patients to stretchers or wheelchairs, as required • Delivers and collects trays and bedside, delivers flowers to patients. • Strips units/cubicles • Performs emergency clean-up • Performs emergency transport • Performs discharge bed making • Transport specimens. • Updates communication board/discharge log • Assembles medical records • Updates Admission/Transfer/Discharge System/Processes • Maintains supplies and care of equipment. B. Organizational/Managerial • Participates in the department’s performance improvement activities. • Maintains patient/employee confidentiality in the management of information. • Observes the Health Care System’s compliance policies. C. Educational/Professional Development • Participates in the development of other staff members. • Meets regulatory, licensure and annual health assessment requirements. • Identifies learning strengths and needs • Utilizes learning resources. D. Communication/Relationships • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. • Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community. Essential Duties and Responsibilities: Provides direct supervision and monitoring of the nursing care given to our residents on a daily basis ensuring required documentation is completed. Provides leadership and guidance to the nursing staff. Provides treatments and medications for our residents. Attends and participates in “at risk” meetings, morning clinical meetings, QAPI meetings, safety meetings, and care plan meetings. Shares “on-call” responsibilities with other supervisory staff and provides direct care as necessary. Investigates accidents and incidents related to our residents and staff. Writes and reviews care plans identifying the problem/needs of each resident, the care to be given, goals to be accomplished and updating the care plan as needed. Conducts nursing assessments. Reviews resident change in condition report daily to ensure follow up occurs including appropriate interventions are implemented and the family has been notified. Complies with and ensures that staff is in compliance with HIPAA rules and regulations. Initiates disciplinary action as necessary for staff members. Quality, Safety and Experience Improvements 1. Responsible for VMS safety, quality idea boards on unit. 2. Accountable for the execution of daily and shift safety and clinical care huddles. 3. Rounds on existing and new patients, addressing issues or concerns of patients and/or families on the unit. 4. Responsible for the closure of Safety Intelligence reports and appropriate Corrective Action Plans in place. Responsible for reporting all Serious Reporting Events (SRE) to leadership and provide feedback to staff on results. 5. Description The Assistant Nurse Manager of ID (Infectious Disease) Transplant / Ophthalmology / Ambulatory Float Pool is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Licensed Practical Nurse (LPN) New Vanderbilt Rehabilitation and Care Center is seeking dedicated Licensed Practical Nurses (LPNs) to join the team. Competitive pay Positive work environment Employee of the month 10% shift differential for evenings and nights Summary of the Licensed Practical Nurse (LPN): Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing. Essential Duties & Responsibilities of Licensed Practical Nurse (LPN): Assess and document resident progress in the medical record. Monitor turning and positioning of residents. Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated. Maintain clean, orderly treatment cart and restock treatment supplies. Follow Infection Control policies and procedures. Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures. Document pressure sore status weekly or more often as condition changes. Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents. Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home. Participate in Quality Improvement activities, as assigned. Job Requirements of Licensed Practical Nurse (LPN): Licensed Practical Nurse current state license in New York. At least one (1) year of Skilled Nursing experience preferred. Must have a current/active CPR certification. Excellent technical, assessment, and documentation skills. Effective verbal and written English communication skills. Customer service oriented with the ability to work well under pressure. Ability to work with minimal supervision, take initiative and make independent decisions. Approachable, flexible and adaptable to change.
