Help patients and families understand and navigate topics such as: Medication administration Hospice philosophy and services Symptom and pain management End-of-life processes and expectations Essential Functions Job Responsibilities Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification. Coordinate care across interdisciplinary teams including consultation with physicians and nurse partitioners. Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs. Communicate change in condition, or signs and symptoms of decline to appropriate provider (e.g., NP, PCP, Hospice Physician). Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. The Clinic RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants. Assists in the examination, testing, and treatment of participants. Implements orders written by primary care providers. Supervises LPN/LVN’s and Medical Assistants in addition to evaluating the overall effectiveness of the clinic in the absence of the Nursing Services Director/Clinic Nurse Supervisor. Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that appropriate staff maintains a list of medical supplies. Effectively communicates and actively engages in interdisciplinary team meetings, family meetings, and clinic meetings. Participates within the interdisciplinary team in the formulation of Plans of Care participants, as well as in other interdisciplinary team settings. Monday through Friday position with no weekends or on-call! InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience. As a Psychiatric Nurse Practitioner (PMHNP) you are an integral part of developing and delivering a personal care plan, ensuring high quality, clinically appropriate, team-based behavioral health care. Physical attendance at an InnovAge center(s) is essential to performing the job duties with the option to travel to meet participants outside of center. No on-call or weekend hours required. The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
