The position requires a reliable vehicle and travel to patients in facilities in Philadelphia and surrounding counties. Responsibilities: - Provide compassionate care and assistance to hospice patients in their homes or care facilities - Assist patients with activities of daily living such as bathing, dressing, and grooming - Assist with physical examinations under the direction of healthcare professionals - Support patients with mobility needs, including lifting and transferring - Maintain a clean and safe environment for patients Qualifications: - Certification as a Hospice Aide or Certified Nursing Assistant (CNA) - Proficiency in medical terminology and understanding of medical office procedures - Experience in providing care for patients in various settings - Ability to accurately measure and record vital signs - Strong interpersonal skills and the ability to communicate effectively with patients and their families - Knowledge of infection control procedures and safety protocols - Physical ability to lift and move patients safely Job Type: Full-time Expected hours: 40 per week Physical Setting: · Long term care · Nursing home · Assisted living · Personal Care · Residential Home Benefits: 401(k) 401(k) matching Health insurance Life insurance Mileage reimbursement Paid time off Work Location: On the road

Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with physician and other health professionals to prepare comprehensive patient care plan. Prescribes or recommends drugs or other forms of treatment. Monitors and maintains progress of patients and reports progress to supervising physician. Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT Qualifications While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required. Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus. Requires completion of accredited Nurse Practitioner program.
