This is a part-time day shift, 32 hours per week position for an RN Case Manager with hospital background. Weekend rotation is required for every 4 th Saturday. We are in search of a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Qualifications: Currently licensed to practice in the State of Ohio as a Registered Nurse ASN required, BSN degree preferred Experience in case management is a plus Excellent knowledge of case management principles Effective communication skills Problem solving skills and ability to multi-task. Med/Surg/ICU experience Responsibilities: Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s health and discharge services needed. Handle case assignments, review cases daily and provide concurrent review Submit necessary cases to the appropriate insurance company for authorization of stay. Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction. Record case information; complete accurately all necessary forms/documentation. Promote quality and cost-effective interventions and outcomes. Ability to multi-task, problem solve with excellent written and verbal skills. Ability to work effectively in a self-directed role. Prior authorizations Run daily reports. Communicate with insurance companies to obtain authorization for proper reimbursement. Adhere to professional standards as outline by protocols, rules, and regulations. All other tasks as assigned.

POSITION OVERVIEW The Certified Assisted Living Associate (HHA) provides care and support to Assisted Living and Assisted Living Memory Care residents, ensuring their comfort, dignity, and safety. The role includes direct patient care, emotional support, and collaboration with healthcare professionals to maintain a positive environment for residents. Key Responsibilities Assists AL residents with daily living activities, including bathing, dressing, grooming, toileting, and mobility. Additionally, assists with medication reminders and ensures AL residents follow prescribed healthcare plans. Monitors and documents vital signs (e.g., blood pressure, heart rate, temperature) as directed by supervising nursing staff. Provides companionship and emotional support to AL residents, promoting a positive and nurturing environment. Assist AL residents during mealtimes, including serving food, feeding if necessary, and ensuring dietary needs are met. Helps with mobility and transfers, using proper body mechanics and safety protocols. Maintains a clean, safe, and organized environment in AL residents’ rooms and common areas. Complies with facility policies, state regulations, and safety guidelines to provide the highest standard of care. Job Details Kettering Health Preble | Eaton | Full - Time | Nights Responsibilities & Requirements The charge nurse leader is responsible for the assessment, coordination, implementation and evaluation of the delivery of safe and effective patient-centered care during their assigned shift. The charge nurse will facilitate, precept, and mentor staff and team members to grow professionally and advance their knowledge and skills. They will continuously assess unit needs to ensure adequate resources for current and upcoming shifts by collaborating with other department, clinical and nonclinical, to coordinate timely admissions, transfers, and discharges of patients. The charge nurse will always demonstrate professionalism, meet regulatory and policy requirements, engage standards of practice, and hold team members accountable for safe, competency practice, quality care, and continual improvement. Charge Nurse reports to Clinical Nurse Manager. JOB REQUIREMENTS: Bachelor’s degree required within 3 years of hire Registered Nurse; Current RN licensure in the state of Ohio Minimum of 1 years clinical nursing experience Work Hours Days or nights with weekend commitment CPR required ACLS, PALS, NRP TNCC per unit requirement (If required and not current must be obtained within 6 months of employment) demonstrated leadership and coaching abilities, demonstrated professional and ethical behavior, must be passionate about nursing and teamwork.
