Ensures complete discharge/transfer documentation including destination, departure details, escort, supplies, medications, and any care training provided. Confirms appropriate medications accompany residents; ensures narcotics are signed out of the narc book. Transcribes orders, AVS, outside provider notes, and care information into the EHR per RJHC policy. Ensures orders are second-checked and placed in RCM boxes for processing. Maintains objective, professional documentation in the resident EHR. Assists with audits, care planning, incident investigations, wound care, and other nursing assessments as assigned during low-volume periods.
