OVERVIEW: Assist the Director of Nursing and Assistant Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: • Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly • Assesses residents/clients upon admission and periodically as conditions change • Performs rounds to evaluate patient condition • Provides or assists with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated • Obtains and implements Physician Orders, checking for accuracy and completeness • Communicates between shifts regarding changes in resident/patient conditions • Ensures a safe and secure environment for patients/residents, staff and visitors • Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients • Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. • Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services • Participate in developing, implementing, and periodically updating the resident’s written discharge plan • Ensure that a sufficient number of nursing staff is available for each shift to ensure that quality care is maintained • Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Ensure safe, timely documentation, patient education, and post-sedation evaluation. · Accurately documents and administers medications as ordered by the provider and provides associated teaching. · Documents competently in the electronic medical record using the correct PPGNY templates and forms. · Arranges for interpreters for patients with communication barriers. · Collaborates with members of a multi-disciplinary health care team & provides care within New York State RN scope of practice. · Assists in promoting optimum center operations, excellent patient service and compliance with both agency and funding source policies and regulations. · Participates in health center/affiliate efforts to achieve established goals for productivity. · Adheres to affiliate goals and policies on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints. · Assists in the orientation and training of new RN/LPN or unlicensed medical personnel on the center team as needed. · Performs other educational and experience related duties as required. · Assists in abortion service in a variety of functions including abortion counseling, · Provides health education and nursing care related to family planning, reproductive health, abortion service, transgender health services, and STI’s to all clients. · Provide accurate patient centered, non-judgmental, education according to MS&G and signs informed consent related to visit. · Performs laboratory tests as ordered, completes laboratory requisitions, packages labs for transport according to PPGNY lab procedures. Performs daily controls per lab protocols. · Follows all safety and security procedures and guidelines. · Participates in agency infection prevention and quality control initiatives and protocols. · Notifies patients of laboratory results and educates regarding clinician plan for follow up. · Screens and follows up on patient phone calls in collaboration with center APC or PPGNY physicians. Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. Description The Clinical Oncology Nurse Navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge, who provides individualized assistance to the patients, families, and caregivers through informed decision making, timely access to quality health and psychosocial care, and coordination of appropriate follow-up visits throughout the cancer continuum. This individual helps overcome healthcare system barriers, influence the systems and behaviors, and address the gaps of care through research and evidence-based practices guided by regulatory committees and professional organizations. The Clinical Oncology Nurse Navigator supports the organizational strategies to deliver high-quality care to the population served through community needs assessment, navigation program development, marketing and outreach, and ONN specific-outcome metrics (i.e., care coordination, guidance, education, and advocacy) established by the organization.
