Primary Care Provider WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with the opportunity to continue living in their homes and in their communities. At the direction of the Medical Director, the Primary Care Provider works collaboratively with WelbeHealth Physicians and the IDT to complete assessments, plan primary care, and educate WelbeHealth participants and their families. Uses organizational skills to keep unit running smoothly and uses prioritization skills to meet deadlines as indicated. Provides assessment data to certified nurse within the scope of practice (vital signs, patient verbal and nonverbal responses, etc.). Performs other job-related duties as assigned. The ideal candidate will be able to provide assistance with daily living activities, manage medical equipment, and promote a safe and comfortable living environment. Responsibilities: Provide transfers using a Hoyer Lift, ensuring the client's safety and dignity. Assist with periodic catheter drainage and maintain proper hygiene practices. Help with changing bandages as needed and maintain a clean and organized environment.
