Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Examples Of Typical Tasks Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. Participates in defining, implementing, maintaining and interpreting standards of nursing practice. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. Participates in determining conditions, resources and policies essential to the delivery of nursing care services. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Performs other professional nursing activities as delegated by nursing service administration. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Job Summary: Provides personal care and support services to clients in a Comfort Keepers established Personal Care Program. Assists the client with activities of daily living by providing any of the Comfort Keepers approved personal care activities including, but not limited to: bathing, dressing, toileting, grooming, oral hygiene, and/or ambulation. Qualifications: Minimum of ninth grade education; high school diploma or GED preferred. Must demonstrate satisfactory completions of any state-mandated training. Applicant must be bondable and meet or exceed minimum qualifications for each of the following background checks: Criminal Background Investigation, Motor Vehicle Driving Record, Credit History, Professional and Personal Reference Checks, and give permission to submit to random drug and alcohol testing. Must have reliable transportation and fulfill assignments with reliability and punctuality. Must have a valid driver’s license and automobile insurance. Must satisfactorily complete Comfort Keepers training and orientation program. Must accept responsibility for learning and adhering to Comfort Keepers policies and procedures, be able to function in the home setting with minimal direct supervision and maintain satisfactory relationships with administrative staff, clients, and family members. Essential Functions: Performs duties as assigned on the personal care Service Plan. Assists the client with activities of daily living by providing any of the Comfort Keepers approved personal care activities including, but not limited to: bathing, dressing, toileting, grooming, oral hygiene, and/or ambulation. Provides for a clean, safe, and healthy environment for clients and family members. Provides light housekeeping tasks including laundering of client’s garments and linens and changing the bed. Assists in meeting nutritional needs by providing meal planning, meal preparation, grocery shopping, dishes, and kitchen clean-up. Assists the client with ambulation/transfers as specified in the Plan of Care. Reminds client to take self-administered medications. Observes and reports any changes in the client’s mental, physical, or emotional condition or home situation to immediate supervisor in a timely manner. Establishes and maintains effective communication and a professional relationship with clients, family members, and co-workers. Participates in in-service and continuing education programs, staff meetings, and client conferences as requested by immediate supervisor. Completed required documentation of services delivered and submits to office in a timely manner according to policy. Uses equipment and supplies safely and properly. Maintains confidentiality regarding client information. Other reasonable related duties as assigned. Working Environment: Client home setting and automobile. Contact with blood or other body fluids may pose a risk for exposure to blood borne pathogens and infectious diseases. Position Physical Demands: The work requires moderate physical exertion on a regular and recurring basis, such as driving, assisting clients in transfer activities and ambulation, and light housekeeping. Regularly required to stand, sit, walk, talk, and hear, reach, kneel, and crouch. Lifting only up to 25 pounds is permitted and may be required.
