Every day you will expertly assess patients pre-procedure, assist physicians during endoscopy, monitor vital signs and sedation levels, and meticulously provide post-procedure care and education, maintaining a sterile and efficient environment. Delivers professional nursing care to endoscopy center patients during all phases of care; adheres to all applicable policies and procedures, including safety, risk management, and regulatory standards. Prepares patients for endoscopic procedure: Performs pre-procedure assessment and gathers data relevant to patient procedure to include, review pre-procedure patient information, history, medications, and allergies. Plans care appropriately based on needs identified during initial and on-going assessments. Addresses more complex, urgent, or emergent situations and escalated from other endoscopy center staff. Performs point of care testing (CLIA-Waived Tests only) following established protocols/instructions. Inserts peripheral intravenous catheter per established protocol. Administer pre-procedure medications as ordered by physician; documents medication administration per established procedures. Provides care to the patient and assistance to the physician during the endoscopic procedure: Assists in preparing the procedure room prior to each procedure, including general cleaning, room turn-over, and restocking of supplies. Determines availability of all necessary equipment and supplies; procures unanticipated items needed for team members during the procedure. Serves as an integral member of the time out process and is directly responsible for completing the pre-procedure checklist. Documents procedure and interactions in patient’s electronic medical record in accordance with established procedures. Assures accurate labeling and ordering of pathology specimens obtained during the procedure. Provides care to the patient during the post-procedure/recovery phase: Monitors patient and records vital signs, level of consciousness, and pain level during recovery. Alerts physician of patient’s status and any adverse events. Determines patient appropriateness for discharge from recovery. Provides discharge education to patient and family/caregiver; provides education regarding results, findings, and diagnoses within scope of practice. Job Summary and Responsibilities This job is responsible for the planning, coordination and placement of patients within the facility. Additionally, hospital supervisors have delegated administrative responsibility and authority for the supervision of hospital personnel and functions within assigned facility, including patient care delivery, in the absence of Administrator or Department leader. JOB DESCRIPTION Hybrid day-shift RN Care Manager position supporting Medicaid members across hospitals in King, Pierce, Snohomish, Kitsap, and Clark Counties. This field-based role requires approximately 75% travel and focuses on case management, discharge planning, and care coordination to ensure members receive appropriate healthcare services and support. Candidates must possess an active RN license and a minimum of three years of experience in case management, discharge planning, or care coordination. Reliable transportation, a valid driver's license, and current auto insurance are required to meet travel demands. Job Summary Provides support for care transition activities. Facilitates transitional care processes and coordination for member discharge from hospital admission to all other settings. Essential Job Duties • Follows member throughout a 30 day program that starts at hospital admission and continues oversight through transitions from acute setting to all other settings, including nursing facility placement/private home, with the goal of reduced readmissions. • Ensures safe and appropriate transitions by collaborating with the hospital discharge planner, as well as collaborating with hospitalists, outpatient providers, facility staff, and family/support network. • Ensures member transitions to setting with adequate caregiving and functional support, as well as medical and medication oversight support. • Works with participating ancillary providers, public agencies or other service providers to make sure necessary services and equipment are in place for safe transition. • Conducts face-to-face visits of all members while in the hospital and, home visits high-risk members post-discharge as needed. • Coordinates care and reassesses member needs using the Coleman Care Transition model post-discharge. • Educates and supports member focusing on seven primary areas (Transition of Care Pillars): medication management, use of personal health record, follow-up care, signs and symptoms of worsening condition, nutrition, functional needs and or home and community-based services, and advance directives. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care, provides care coordination and assistance to member to address concerns. • Facilitates interdisciplinary care team meetings (ICT) and collaboration. • Provides consultation, recommendations and education as appropriate to non-behavioral health care managers. • 40-50% local travel may be required (based upon state/contractual requirements).
