Initiates and/or participates in patient care conferences. Assumes the responsibility to interact on a consistent basis with the patient and family providing coordination of the plan of care for those patients whose needs do not require the expertise of a Case Manager. Promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of neonatal, pediatric, adolescent, adult and geriatric patients.
