While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population. We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities. NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with OH Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Ohio. Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. Job Description Seeking full-time or part-time Psychiatric Mental Health Nurse Practitioner or Physician Assistant in the Sidney, Vandalia and Dayton, Ohio area. The responsabilities will be to provide rapid growth and quickly expanding psychiatric service obligations to residents in long term care facilities. This skilled professional must be able to work collaboratively with members of interdisciplinary treatment team to develop, implement or evaluate treatment plans. We are seeking a skilled professional that is committed to providing these services with a high level of integrity and thrives for excellence in the care of the residents. Minimum Job Requirements: Licensed nurse in the state of Ohio with Advance Practice Registered Nurse License in good standing. or Physician Assistant degree is the State of Ohio. Hold a degree as a PMHNP - Psychiatric Mental Health Nurse Practitioner at the level needed to assess and prescribe medication collaboratively with a psychiatrist as determined by the State Board of Nursing. Or a Physician Assistant with experince in the psychiatry field. Board Certified Psychiatric experience required. Geriatric experience preferred. Active DEA required.
