PRIMARY FUNCTION The Admissions Registered Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the admission of appropriate patients. JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care.
