Includes the patient and the family in the planning process. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilize opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group Qualifications: Graduate from an accredited school of nursing. Bachelor’s degree preferred. Current state license as a Registered Nurse. Minimum of 1 year of recent acute care experience in an institutional setting. Minimum of 1 year of recent experience in home care, preferably in hospice. Rancho's Neonatal Intensive Care Unit (NICU) is operated by Rady Children’s. Southwest Healthcare Inland Valley Hospital in Wildomar, California , is a licensed 122-bed facility accredited by the Joint Commission. It is Southwest Riverside County's only designated Trauma Center and a county-designated Paramedic Base Station. The hospital performs more than 4,000 surgeries a year, and has an average of 4,000 Emergency Department visits per month. LVN License Required POSITION SUMMARY The primary purpose of your job position as MDS Nurse is to provide direct care to residents. In your role as MDS Nurse, you will work to restore or maintain the resident’s health and well-being by assisting the MDS Coordinator with health and wellness problem identification (actual/potential), goal setting within the patient care plan, and establishing appropriate medical interventions. Ensure that direct care is provided to residents including, preventive measures, treatments, medications, diet and other health services prescribed by the attending physician are promptly carried out and documented, and recommend and implements changes in the administration of direct care, treatment methods, and care plans.
