Job Description The Division of Geriatrics seeks a per-diem Nurse Practitioner to support the Age-Friendly Geriatrics Emergency Department (AFED) consult service. UCSF is engaged in providing care based on “What matters” as part of the Age-Friendly Health System (AFHS). This position requires optimizing the care of older adults presenting to the emergency department setting when geriatric consultants are requested by ED providers. The clinical work involves providing consult services from Geriatrics to ED by completing structured assessments, patient exam, and making recommendations to the ED and Primary Care Physicians (PCPs) for ongoing care/ home management as indicated.
