Job Description Under general supervision, guided by the policies and procedures of the Ambulatory Services, and in collaboration with the physician staff, the Clinical Nurse II performs established nursing procedures using clinical knowledge to care for patients. Qualifications Required Qualifications: One or more years of clinical hemodialysis experience (outpatient or inpatient) within the last 12 months. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
