How you make a difference As a Nurse Practitioner MAT, you will deliver comprehensive medical care to patients, operating within established policies and procedures and in alignment with state regulations, under the guidance of the Site Medical Director and fellow site Physicians. Your role involves meticulous documentation of provided services for monitoring purposes and promptly communicating any schedule changes or coverage needs to the Medical Director and site director to ensure uninterrupted medical services for patients even during your absence. Key Responsibilities Conducts thorough medical examinations of referred patients upon reporting to the assigned facility, adhering to clinic treatment policies and all relevant state and federal regulations. Provides guidance and training to medical staff, participates in case conferences, and collaborates with clinic personnel on treatment plans while ensuring accurate and timely documentation of patient progress in SOAP format. Responds promptly to health emergencies following standard protocols, reviews records as necessary, and utilizes available resources for problem resolution before considering off-site referrals. Performs comprehensive medical assessments, including history-taking and physical examinations, to diagnose opioid dependence and coexisting medical or psychiatric disorders, making appropriate referrals for further treatment. Prescribes tailored patient care regimens, upholding confidentiality standards and contributing insights to facility committees upon request from the Site Medical Director or Health Services Administrator. Additional Details ** Part-time: 24 hrs/week on Mon/Wed/Fri ** Qualifications & Requirements Education Graduate of an accredited School of Nursing. Advanced degree in an approved and accredited Nurse Practitioner program. Experience 2 years’ experience as a Nurse Practitioner Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification. Must have or be willing to obtain DATA 2000 Waiver prior to employment. The role conducts basic nursing assessments, reviews health records, coordinates referrals, and facilitates warm handoffs to managed care plans and community providers. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. How you make a difference The Nurse Supervisor is responsible for the direct supervision of the health care delivery system within their assigned nursing unit. They collect health status data, diagnose patients, set goals, plan and implement care, and evaluate patient progress. The Nurse Supervisor also provides patient education and ensures that ancillary personnel are qualified to perform the services they provide. Key Responsibilities Review and ensure compliance of nursing plans, medical records, and referrals with policies and procedures. Monitor and facilitate progressive patient care through team approach and patient education. Assist DON in maintaining clinical skills of registered nurses and ensuring nursing actions are within scope of licensure. Identify and address potential problems, establish priorities, and propose corrective actions. Maintain unit cleanliness and coordinate patient care with other departments, while ensuring appropriate stock levels of medical supplies and equipment maintenance. How you make a difference The Nurse Discharge Planner LVN develops and coordinates clinically informed reentry care plans for CalAIM-eligible individuals to address medical, behavioral health, and substance use needs prior to release. The role conducts basic nursing assessments, reviews health records, coordinates referrals, and facilitates warm handoffs to managed care plans and community providers. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. Facilitate psychoeducational sessions, benefit applications, and resumption of services to support continuity of care. Maintain working relationships with facility staff, parole, mental health, and medical partners while adhering to security regulations. Document services in the health record, activate Justice-Involved aid codes, and ensure compliance with DHCS CalAIM requirements. Qualifications & Requirements Education Graduation from an accredited School of Nursing is required. Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred Experience Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred. Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Must maintain CPR certification, and any other certifications (such as First Aid) required by contract. Must be able to attain and maintain security clearance to the facility. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Conducts routine and emergency medical care for inmates, including chronic care clinics, infirmary rounds, and emergency treatment on-site. Adheres to established policies and procedures for medical care, including documentation in SOAP format, adherence to formulary regimens, and utilizing available in-house resources for treatment. Assumes responsibility and accountability for individual judgments and actions, while also reporting any concerns or questions to the Medical Director or Health Services Administrator. Maintains patient confidentiality and privacy in accordance with accepted practices for incarcerated individuals. Provides input into facility mandatory committees as requested.
