Three core responsibilities of the Admission Case Manager are to provide: Consultation and direction for physicians about the appropriate determination of the level of care, specifically Observation vs. Inpatient status. Discharge planning directly from the ED to levels such as Acute rehab, SNF, and homecare services. Development and implementation of care plans for those patients who visit the ED frequently.
