The Registered Nurse utilizes the nursing care process consistent with their license and/or certification including initiation of culturally competent continuum of care including inpatient, outpatient and community settings. The Registered Nurse is responsible for overseeing any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. The patient care process includes: * Assessment - an orderly process consisting of the identification, gathering and organization of subjective and objective data pertaining to the patient/family/significant other, including physical, psychosocial, spiritual and cultural. * Planning - an organized step in the process that consists of the identification of actual or potential problems and desired outcomes (goals) that result in a documented plan of care. The plan of care provides direction for the patient/family/significant other and interdisciplinary team. * Intervention - the specific interventions or orders that are implemented consistent with the plan of care and medical treatment plan. * Evaluation - a step in the process that involves determining the effectiveness of the interventions in achieving the plan of care and determining the conclusions concerning the achievement or lack of achievement of desired outcomes. The evaluation may initiate the need for reassessment and revisions to the problems, goals, plan of care, and interventions. This step provides for documentation of required actions. Based upon findings identified during assessment and reassessment: * Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self-care, educational and social status. * Goals are identified consistent with the problem to provide direction for patient care and health promotion. * Education is consistent with the identified problem and associated goals and is seen as critical to creating an environment that promotes patient empowerment * Employee may be required to perform Limited Laboratory Services to include patient identification and preparation for specimen collection, processing samples for testing, performing required quality control testing and instrument maintenance and function checks, monitoring, recording and reporting tests results to clinicians to include critical values, and maintaining competency/certification of the aforementioned tasks and duties. Licensed and registered care providers responsible for utilizing the care process relative to their scope of practice and knowledge base, may document the assessment, plan, intervention and Evaluation. QUALIFICATIONS EDUCATION/LICENSE: * Graduate of an accredited school of nursing; BSN preferred. * Current Licensure as a Registered Nurse in the State of New Jersey or current multi state license required. * American Heart Association BLS required upon hire or transfer. EXPERIENCE: Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire. Essential Functions : Attend work as scheduled and follow company’s call-off procedures · Follow established performance standards and perform duties according to nursing service policies and procedures. · Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse. · Complete assignments timely, completely and accurately. · Conduct resident rounds as assigned. · Initiate corrective action as necessary and/or seek assistance of a licensed nurse. · Attend all in-service classes as assigned and complete assignments. · Document incidents and/or unusual problems according to established facility procedures. · Review care plans and daily assignments and perform nursing care as outlined. · Document in the nursing assistant notes the care and treatment provided to the resident and the resident’s response or lack of response to care provided. · Report changes in residents’ conditions to Charge Nurse or Nurse Supervisor. · Follow facility procedures for admission, discharge and transfer of residents. · Complete documentation accurately, timely and following facility policies and procedures. · Listen to resident and family complaints and report problems to Charge Nurse. · Assure residents have call lights at hand and answer call lights promptly. · Assure residents are given adequate assistance with personal and oral hygiene needs following facility’s policies and procedures. · Assist residents with activities of daily living as documented in the residents’ care plans. · Assure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s). · Check name bands on all assigned residents daily and report to Charge Nurse any bands that have been removed or need to be changed. · Observe infection control procedures. · Be responsible for the safety of residents under his/her supervision including the use of safety devices and/or alarms. · Protect the privacy of resident Protected Health Information. · Protect the confidentiality and security of all resident and facility information.
