Summary Baylor College of Medicine (Houston, TX) is seeking an Advanced Practice Provider (APP) for the Section of Hematology/Oncology and the Dan L Duncan Comprehensive Cancer Center (DLDCCC), one of only three National Cancer Institute Comprehensive Cancer Centers in Texas. The Classical Hematology APP will play a pivotal role in the DLDCCC for the classical hematology patients. The successful candidate will work closely with our classical hematologists to provide comprehensive care for patients with benign/non-malignant hematological disorders. This APP's effort will be devoted to patient care in the classical hematology clinics. Job Duties Performs comprehensive health assessments, physical exams. Performs, orders, and interprets diagnostic procedures in accordance with relevant health concerns of the patient. Evaluates and manages on-treatment and post-treatment side effects, offers treatment recommendations and prescribes medications to manage symptoms of treatment; collaborates with infusion center for iron infusion and other treatments as needed to support coordination; plans and implements therapeutic regimens that include both nursing and medical management strategies, including prescription of medications. Provides monitoring and continuity of care between visits. UTHealth Neurosciences houses one of the largest and busiest neurosurgical programs in the country at Memorial Hermann TMC. We are currently looking for a PRN Advanced Practice Provider (APP) to support our neurosurgical service alongside our world-renowned neurosurgeons, neurosurgical residents, and fellows . The schedule is flexible and on an as-needed basis. The APP will be assigned one primary neurosurgeon and will see patients primarily in the outpatient setting. There will be some minor inpatient coverage during normal business hours (primarily helping with discharge, seeing established patients who present to the ED, answering patients’ questions via MyChart and by telephone, and assisting with care coordination). We are currently looking for a full time Advanced Practice Provider (APP) to support our neuro trauma service along side our world renowned neurosurgeons and neurosurgical residents . The APP works Monday through Friday, no nights/holidays/weekends. The hours are 530-1330. The APP will see all the neurosurgical consults that come to the hospital, answer phone calls for the service line, and assist the team with the initial evaluation and assessment of the neurotrauma patients in the ED or as direct admits to either the ICU, IMU, or floor. This APP will be trained to perform lumbar punctures, lumbar drains, craniotomy for insertion of ICP bolt monitor and/or ventricular catheters, scalp wound closures, removal of drains, and consenting patients for procedures and speaking to families about the patient's condition. Concierge Nurse Navigator - TCP Medical Group Clinic

Job Description We are searching for a Concierge Nurse Navigator at the TCP Medical Group clinic someone who works well in a fast-paced setting. In this position, you will deliver comprehensive clinical, operational, and administrative support to ensure high-quality patient care and efficient practice operations. Support daily workflow, managing medical supplies, ensuring compliance with regulatory standards, and maintaining accurate financial and membership records. Provide direct patient care through preparing patients for examinations, assisting providers during procedures, administering medications, collecting specimens, performing laboratory tests, and coordinating referrals. Job Duties & Responsibilities Provides Daily Operations Monitors and ensures patient flow Prepares deposit weekly and assures daily work is balanced and ready for pickup by bank courier Collects and disseminates mail and deliveries per practice protocol Participates in interdisciplinary meeting to promote and ensure adjustment and continuity of patient care Prepares patient for examination per practice protocol Completes treatments/lab as directed by provider and within scope of position Triages phone calls per practice protocol and within scope of practice Documents all clinical patient care calls and interventions per organization recommendations Participates in quality improvement initiatives that support patient care Verifies provider order and patient per practice protocol Collects and processes specimen according to manufacturer’s instructions and using proper technique Coordinates patient care by following up on lab results, ER visits, or any part of care that needs coordination Coordinates referrals Reviews billing information Monitors TCP Concierge Self-Pay Workqueue on a weekly basis and corrects within seven (7) business days of delinquency Skills & Requirements Diploma or GED And Other Diploma or Certificate from a school of vocational nursing Required OR Documentation from the Texas Board of Nursing verifying an exception to the requirement for a diploma from a school of vocational nursing LVN - Lic-Licensed Vocational Nurses State of Texas BLS - Cert-Basic Life Support American Heart Association 1 year LVN experience required 1 year pediatrics exp preferred 1 year Nurse Navigation expeience in a specialty perferred About Us Texas Children’s Pediatrics is the nation’s largest primary pediatric care network with more than 200 physicians dedicated to the health care needs of infants to teenagers in over 50 locations throughout the greater Houston area.
