Summary Responsible for providing care to clients in their homes which include light housekeeping, meal prep, assistance with daily living, medication management, memory care support, and help with many other special needs. Assists clients with activities of daily living (ADLs), such as bathing, toileting, dressing and personal hygiene, ambulation, and household safety. Accompanies client to appointments and local trips. Performs general housekeeping including cleaning, laundry, and meal preparation. Redirects clients with memory issues; providing safety guidance protection to the client who cannot be left alone due to impaired judgment. Maintains excellent and objective ongoing communication with the SHC office staff; respond to phone calls and/or emails in a timely manner. Administers medication with nurse supervision and track medication on a Medication Administration Record. Provides companionship, including activities, discussions, encouragement, reassurance, and redirection as needed. Required Skills/Abilities Consistent execution of quality customer service . Duties include pushing wheelchairs over varying distances, occasional overhead reaching, and periodic twisting, bending, turning, and squatting. Places vascular access devices such as: peripheral intravenous catheters (PIV), Peripheral Inserted Central Catheters (PICC), Midline catheters and Intra-osseous catheters. Accesses and de-accesses implanted ports. Assesses and manages central venous access devices including routine dressing changes, cap changes and intervenes to maintain patency of central venous access devices. Serves as a resource for the health care team in the intravenous access, placement, management, and patient/family education for care and maintenance. Service area is both inpatient and outpatient. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
