Become knowledgeable of each individual resident’s background, cultural, life history, disease, and medical care needs in detail to ensure an appropriate person-centered activity plan. Activities shall be daily and shall reflect the schedules, choices, and rights of the residents. Be able to understand communication needs of residents, such as language services, communication boards, etc. Per direction of the Activity Director, conduct, oversee, and complete initial and all on-going assessments, including but not limited to social, medical, cognitive, communication abilities, spiritual needs, physical, neuro-psychological, behavioral, amongst others as it relates to individual activity/leisure. Document and complete all required documentation per Activity Director. Maintain the following records: Resident Daily Participation, Resident Progress notes, Leisure Interest Survey, and Activity Planning sheets to assist in scheduling and meeting resident needs. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Must maintain equipment and supplies in good clean order. Must notify Activity Director within 48 hours if any new materials are needed for scheduled activities and when repairs are needed. Must update calendars and boards daily. Perform other duties as assigned.

Recommends to the Director of Nursing the equipment and supply needs of the department. Inventories and stores the new customer’s personal possessions. Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing. Assist the resident and Discharge Planning Coordinator in planning the nursing portion of the resident’s discharge plan. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. Monitor the facility’s QI, QM and survey reports and provide the Director with recommendations that will be helpful in eliminating problem areas. Assist the Director in obtaining staffing information that must be posted on a daily basis. Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to Administrator. Service on the Quality Assurance and Assessment Committee as directed. Schedule, attend, and participate in developing the agenda for the Care Plan Team Meetings as required. Assist the Director in determining the staffing needs of the nursing department. Recommend to the Director the number and level of nursing personnel to be employed. Delegate to the Charge Nurse and Nurse Supervisors the administrative authority, responsibility, and accountability necessary to perform their assigned duties. Assist the Director in preparing performance evaluations as directed. Make daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to Director. Review complaints and grievances made or filed by department personnel. Make appropriate reports to Director as required or as may be necessary. Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status. Interpret the department’s policies and procedures to personnel, residents, visitors and government agencies as necessary. Ensure that all nursing personnel participate in the facility’s TB testing program. Participate in the interviewing and selection of residents for admission to the facility. Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc. Ensure that rooms are ready for new admissions. Greet newly admitted residents upon admission. Escort them to their rooms as necessary. Review nurse’s notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes. Schedule daily rounds to observe residents and to determine if nursing needs are being met. Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled. Report medication errors to the Director. Provide direct nursing care as necessary. Report problem areas to Director. Assist in developing and implementing corrective action. Participate in developing, planning, conducting, and scheduling in-service training that provide instructions on “how to do the job” and ensure a well-educated nursing department. Participate in the mandatory new hire orientation. Assist Director in scheduling annual mandatory in-service training for staff. (OSHA, HIPAA, Abuse Prevention, Infection Control, etc.) Assist the Safety Officer in developing safety standards for the nursing department. Assist the Director of Nursing in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded. Ensure that nursing personnel work areas (nurse’s station, medication rooms, etc.) are maintained in a clean and sanitary manner. Ensure all resident care rooms, treatment areas, etc., are maintained in a clean, safe and sanitary manner. Ensure that all nursing personnel participate in and conduct all fire safety and disaster preparedness drill in a safe and professional manner. Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained on premises at all times to adequately meet the needs of the residents. Assist the Maintenance Director in developing a preventive maintenance program for maintaining the nursing department’s equipment and supplies in a safe and operable manner. Ensure that only trained and authorized personnel operate the department’s equipment in a safe manner. Participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident. Assist the MDS/CPC in planning, scheduling, and revising the MDS, including the implementation of RAPs and Triggers. Encourage the resident and his/her family to participate in the development and review of the resident’s plan of care. Be sure that staff members are providing care that reflects the wishes of the resident. Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Ensure all nursing personnel are knowledgeable of the residents’ responsibilities and rights include the right to refuse treatment. Performs other duties as assigned.
