Overview Medical/Surgical (ACE) 3-North: Joining this forty-two-bed unit can be a very rewarding step to initially pursue, or to further expand on your already established career in med/surg nursing. With a patient population encompassing mostly medical patients, with roughly 65-70% of the patients being geriatric, the educational and professional opportunities abound. Caring for the elderly population has the potential to bring both personal and professional satisfaction and growth, as this subset of patients have a lot to offer from every facet of nursing. CentraState Medical Center currently has an employment opportunity available for a Clinical Leader in the Medical/Surgical (ACE) 3-North unit. The Clinical Leader plays a pivotal role in coordinating culturally sensitive care from pre-admission through post-discharge. This includes managing resources to achieve optimal clinical and financial outcomes, supporting quality service delivery, and guiding staff in evidence-based nursing practice. The role also assists the Nurse Manager in overseeing unit operations on a 24-hour basis and serves as a clinical resource and educator to team members. Care is delivered through a multidisciplinary, patient-focused model aligned with CentraState’s organizational goals and standards. Responsibilities Assists the Department Head in the continuing effort made by the department to improve the quality of patient care and services. Provides considerate respectful care focused upon the patients' individual needs. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the patients cared for. Demonstrates an understanding of the range of care needed based on the age of the patients cared for. Successfully completes life-span education as appropriate. Successfully completes life-span education as appropriate. Obtains a complete nursing history and baseline assessment that is systematic, consistent, and continuous. This assessment is communicated effectively and documented. Appropriately analyzes health status data to determine nursing diagnosis and needs for appropriate nursing care, medical intervention or services of other clinical professionals. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medications as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Attends and participates in the multi-disciplinary discharge planning meetings and identifies the need for further services following discharge. Provides appropriate education based upon teaching plan. Conducts unit based education programs on a routine basis in collaboration with professional development. Meets standards for documentation regarding patient education. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in Medical Records, Nursing Standards of Care/Practice for the specific patient type. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Delegates components of care to non-professional caregivers as appropriate and evalutates outcomes. Makes decisions that demonstrate knowledge of nursing practice. Utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the achievement of maximum outcome. Develops new clinical pathways, monitors patient progress on current clinical pathways, reviews variances with Patient Care Reviewers, reviews variances from Quality Management with the Director/Nurse Manager and staff and reports to Nursing Quality Council. Follows the clinical progress of all patients. Participates in the development of unit protocols as needed. Conducts unit conferences to discuss quality issues and quality improvement strategies. Quality Referrals - unit based, follows up on nursing referrals, investigates problem, develops action plan, reviews with Nurse Manager, responds to Quality Management in writing, implements action plan. Follows up on patient complaints or concerns and keeps Director/Nurse Manager informed. Provides or coordinates at a minimum, 1 formal in-service per year to address unit specific needs. Monitors appropriate usage of speciality and Zonaire beds. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy # 4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Takes a patient assignment when necessary. Qualifications Bachelor's Degree in Nursing required. Master's Degree in Nursing required or must obtain within three years from date of hire (or internal transfer) as a condition of employment. Two years of Nursing experience required. Supervisory experience preferred. NJ RN License required. CPR Certification required. ACLS Certification to be achieved within first year. Successful completion of Basic Dysrhythmia course to be achieved within first year. CSMC IV Certification to be achieved within first year. Certification in specialty preferred. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team. Director of Nursing (DON) – Skilled Nursing Facility (SNF) Location: Elizabeth, New Jersey Job Type: Full-Time Position Summary We are seeking an experienced and compassionate Director of Nursing (DON) to lead and oversee all nursing operations at our Skilled Nursing Facility in Elizabeth, NJ. The Director of Nursing is responsible for ensuring the delivery of high-quality resident care, maintaining regulatory compliance, managing nursing staff, and fostering a culture of excellence and accountability. The ideal candidate is a strong clinical leader with a thorough understanding of long-term care regulations, staff development, quality improvement initiatives, and resident-centered care. Key Responsibilities Direct, supervise, and coordinate all nursing services within the facility. Ensure compliance with federal, state, and local regulations, including NJ Department of Health requirements. Develop and implement nursing policies, procedures, and standards of care. Recruit, train, mentor, and evaluate nursing staff, including RNs, LPNs, and CNAs. Oversee staffing schedules to ensure appropriate coverage and quality resident care. Monitor clinical outcomes and lead quality assurance and performance improvement (QAPI) initiatives. Collaborate with physicians, therapists, department heads, residents, and families to ensure comprehensive care planning. Manage infection prevention and control programs. Minimum of 3–5 years of nursing leadership experience in a Skilled Nursing Facility, Long-Term Care, or Rehabilitation setting. Previous experience as a Director of Nursing, Assistant Director of Nursing (ADON), or equivalent leadership role preferred. Strong knowledge of CMS regulations, state survey processes, and long-term care operations. Demonstrated leadership, organizational, and communication skills. Ability to effectively manage staff, budgets, and clinical programs. CPR certification required.
