On the HKU, patients range in age from neonates through adults. The HKU primarily admits patients from the Cardiac Intensive Care Unit, Ambulatory Clinic, PACU and EMTC. The unit also admits 23-hour patients with cardiac and renal disease. About The Role All nursing practice is based on the legal scope of practice, national and specialty nursing standards, Children's National Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements. Document any updates, care progression and barriers to discharge daily, and as indicated on assigned patients. Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays and or days. Collaborate with Social Worker to coordinate, long-term care, assisted living, financial assistance, and other services, as required. Send referrals/communicate with in-network vendors for coordination of post-acute levels of care such as Home Health, DME, IV infusion, SNF, Sub-Acute and Acute Rehab. Timely identification, recording, and escalation of delays in care and barriers to discharge. Provide solutions to correct delays and recognize systemic patterns that require corrective action. Assure follow up appointments and referrals to ambulatory case manager for high-risk patient population are scheduled and communicated to patient/family prior to discharge. Observe all facility safety policies and procedures (infection control, Members Rights policies, and any regulatory requirements) Participate in Quality Assurance duties and implementation of programs to improve care Quality Indicators. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES: -Demonstrates knowledge of age specific differences in the patient population including physiological and developmental differences unique to each group. -Assess and monitors therapy of patients using appropriate technical skills under the direction and supervision of a physician. -Assists physician with exams and procedures. -Maintains efficient and appropriate patient flow. -Demonstrates knowledge of reimbursement procedures and charge capture. -Performs venipuncture, accesses implanted ports and maintains central venous access devices and alternate delivering systems. -Documents all elements of care. -Communicates all pertinent information and any interventions to the physician or mid level provider. -Serves as a resource person to the medical assistants and LPNs. -Triages patient/family phone calls as needed and documents relevant information in the patient's chart. -Assesses patient/family needs and provides teaching. -Demonstrates awareness of community resources available to patients and assists with referrals if necessary. -Provides nursing coverage for the satellite offices as needed. -Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. -Maintains current knowledge of oncology nursing by participating in ongoing educational opportunities. -Participates in professional development activities and maintain professional affiliations. -Maintains patient confidentiality. -Serves as mentor/preceptor and provides day to day guidance to less experienced nurses. Qualifications Minimum Job Qualifications (Knowledge, Skills, & Abilities): Graduate from an accredited program for professional nursing education, BSN preferred. Minimum of 3 years nursing experience, preferably in an oncology environment. Current or pending Oncology certification preferred. Experience and knowledge of the purpose and use of medications in the treatment of cancer is preferred. Current RN state license with the State Board of Nursing in practicing state. Current CPR certification required. Certified Medication Technician Job Description Job Summary Performs nurse aide duties that provide for the physical and emotional needs of the assigned patients. Essential Functions Adheres to RAH policies and Maryland Nurse Practice Act. Performs work under the supervision of the Supervising Nurse/Delegating Nurse. Performs nurse aides duties, not limited to: personal care of the patient (bathing, d ressing, or hygiene, toileting activities and feeding), transferring and ambulation activities, range of motion exercises, vital signs (temperature, pulse rate, respiration rate, and blood pressure), and post mortem care. Receives direction from licensed nurses in planning and carrying out nursing aide care. Observes and reports to the licensed nurse the physical and psychological conditi on of the patient including any changes. Reports safety hazards and unusual incidents involving patients, visitors or self to the Supervising Nurse and to the o ffice. Performs the following, provided that he/she has been trained, demonstrated competency and it is within the RAH policy and state law; colostomy care, wound care, catheter care and medication administration. The Certified Medication tech will NOT perform any of the following: taking or transcribing physician orders activities outside the Scope of Practice for CMT as outlined in the State Practice Act.
