Baystate Medical Center now uses CardioMEMS, a new technology that reduces hospitalizations and improves quality of life for heart failure patients. Hospice of Western & Central Massachusetts is seeking a compassionate and experienced Hospice RN Case Manager to join our team! As a key member of our care team, you will provide exceptional hospice care to patients and their families, building relationships and delivering comprehensive services that support their unique needs. Shift is Monday through Friday 8a-4:30p. This is an on-the-road position with mileage reimbursement. Up to $49 an hour with weekly pay.
